POSITION DESCRIPTION
PRN ON CALL REGISTERED NURSE
Position Summary
We are seeking a compassionate and experienced PRN On-Call Registered Nurse (RN) to join our hospice team. The On-Call RN provides skilled nursing support to hospice patients and their families during after-hours periods, responding to urgent clinical needs, symptom-management concerns, and changes in patient condition.
The ideal candidate is clinically confident, compassionate, responsive, and able to work independently. Hospice, after-hours, and/or telephone triage experience is strongly preferred.
Essential Responsibilities
- Respond to patient and family calls during assigned on-call hours and provide appropriate clinical guidance.
- Perform telephone triage and assess patient symptoms and changes in condition.
- Determine the appropriate level of intervention based on the patient's goals of care and hospice plan of care.
- Make scheduled and urgent PRN visits to patients when clinically indicated.
- Assess pain, symptoms, medications, wounds, and overall patient status.
- Provide skilled nursing interventions and symptom-management recommendations.
- Communicate significant changes in patient condition to the attending physician, hospice medical director, and interdisciplinary team as appropriate.
- Collaborate with caregivers and provide education, reassurance, and emotional support.
- Coordinate with hospice staff, facilities, pharmacies, physicians, EMS, and other healthcare providers as needed.
- Respond appropriately to end-of-life situations and provide support to patients and families during the dying process.
- Complete accurate and timely clinical documentation in accordance with hospice, regulatory, and organizational requirements.
- Follow all hospice policies, procedures, standards of care, and applicable federal and state regulations.
- Maintain patient confidentiality and uphold HIPAA requirements.
- Participate in quality improvement and other clinical activities as requested.
Qualifications
Required:
- Current, unrestricted Registered Nurse (RN) license in the state of practice.
- Current CPR certification.
- Minimum of 1 year of professional nursing experience.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and make sound clinical decisions.
- Reliable transportation and ability to travel to patient locations when required.
- Ability to work flexible on-call hours, including nights, weekends, and holidays.
Preferred:
- Hospice or palliative care experience.
- Telephone triage experience.
- Previous after-hours or on-call nursing experience.
- Home health experience.
- Experience with end-of-life symptom management and family support.

- Strong knowledge of nursing assessment and clinical interventions.
- Ability to recognize changes in patient condition and respond appropriately.
- Excellent telephone communication and triage skills.
- Ability to remain calm and professional in urgent or emotionally sensitive situations.
- Strong organizational and time-management skills.
- Ability to prioritize multiple patient needs effectively.
- Compassionate approach to patients and families facing end-of-life care.
- Proficiency with electronic medical records and clinical documentation.
