Full Job Description

PRIMARY FUNCTION

The YouthBuild Program Case Manager is responsible for providing comprehensive case management and supportive services to participants enrolled in the YouthBuild program. Working closely with program staff, educators, employers, and community partners, the Case Manager assists participants in overcoming barriers to education, employment, and personal success. The Case Manager develops individualized service plans, coordinates referrals, monitors participant progress, and supports participants through enrollment, program completion, and post-exit follow-up.

KEY ROLES (Essential Job Responsibilities)

· Conduct participant intake, assessments, and eligibility determination.

· Develop and maintain Individual Service Strategies (ISS) and service plans.

· Provide ongoing case management and supportive counseling.

· Assist participants in setting educational, career, and personal goals.

· Monitor attendance, academic progress, worksite performance, and program participation.

· Coordinate referrals for supportive services, including:

o Mental health counseling

o Housing assistance

o Transportation

o Childcare

o Substance abuse treatment

o Healthcare services

· Maintain regular contact with participants through individual meetings, phone calls, texts, and home visits as appropriate.

· Collaborate with instructors, construction trainers, career coaches, employers, and community partners to support participant success.

· Facilitate workshops on life skills, financial literacy, conflict resolution, and employability.

· Assist participants with postsecondary education enrollment, employment placement, and retention.

· Track participant outcomes and maintain accurate case notes and documentation in the program database.

· Ensure compliance with YouthBuild, Department of Labor, and organizational policies.

· Participate in staff meetings, trainings, and program events.

· Support participant recruitment, retention, and follow-up services.

· Perform other duties as assigned.

RELATIONSHIPS

Internal: Works in close partnership with the YouthBuild Program Director, College & Career Readiness Program Director, the Recruitment & Placement Coordinator, and various departments as appropriate.

External: Maintain contact with external vendors, volunteers, corporate partners, and other groups to achieve goals and objectives.

SKILLS/KNOWLEDGE REQUIRED

· Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Education, Criminal Justice, or related field. Relevant experience may substitute for education.

· Two years of experience providing case management, workforce development, youth services, or social services.

· Experience working with disconnected or opportunity youth.

· Knowledge of trauma-informed care, motivational interviewing, and strengths-based case management.

· Strong communication, organizational, and documentation skills.

· Ability to build positive relationships with diverse youth populations.

· Bilingual (Spanish/English or other languages) preferred.

· Excellent interpersonal and relationship-building skills.

· Ability to manage multiple cases while meeting deadlines.

· Strong problem-solving and crisis intervention skills.

· Cultural competence and commitment to equity and inclusion.

· Ability to maintain confidentiality and professional boundaries.

· Experience working with youth affected by homelessness, justice involvement, foster care, or other barriers.

The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. Vision abilities required by this job include close vision.

Location: Greater Stuart Club

Schedule: Mon – Fri between 8am-5 pm; Hours may vary depending on the location or situation of the day; Occasional weekend work may be required.

Compensation: $60,000.00 per year
