Paygrade: B14 ($90,700 - $137,200 annually, depending on experience)

DESCRIPTION
Plans, directs and supervises programs and activities involved in providing public health services to the community, including but not limited to immunizations/VFC, WIC, Family Planning, Babies First, CaCoon, tobacco prevention and education, environmental health, emergency preparedness, communicable diseases, Oregon Mothers Care, perinatal hepatitis b program and maternal child health. Additionally, oversees the Rural Health Clinic and associated primary care functions.

Work is performed under the general direction of the Executive Director and Grant County Judge.
Supervision Exercised
Supervision is provided to all programs of the public health department and rural health clinic.
RESPONSIBILITIES
-
Develop, plan and implement a variety of programs to provide public health services to the community. Prepare annual departmental health plan outlining and describing program activities, staffing and associated expenditures.

Ensure that the overall operation of the health department is consistent with the agency mission, policies and goals.

Ensure that the agency is well thought of and highly respected.

Develop and sustain a working environment that encourages commitment to provide the highest quality of care/services, integrity and respect for peers, clients and citizens of Grant County.

Work with the Executive Director to plan agency goals and objectives regarding the delivery of services.
-
Ensure that the Executive Director has appropriate and accurate information for decision making processes.
-
Assist the Executive Director in developing and maintaining collaborative relationships with county commissioners, child welfare, schools, and other community partners.
-
Maintain necessary records and prepare reports related to departmental programs and activities.
-
Provide Grant County Commissioners information concerning the activities of the local health department and submit an annual profit/loss document.
-
Act as the agent of the Oregon Health Authority in enforcing state public health laws and rules of the authority, including such sanitary inspection of hospitals and related institutions as may be requested by the authority.

Adopt and update as necessary a local public health modernization assessment.
In consideration of the local public health modernization assessment, adopt, implement, monitor, evaluate and modify as necessary a local public health modernization plan that includes:
a. A plan for applying the foundational capabilities established under ORS 431.131 and implementing the foundational programs established under ORS 431.141 as required by ORS 431.417; and
b. Any other local public health program or activity that the local public health authority considers necessary to protect the public health and safety;
Coordinate with coordinated care organizations and Early Learning Hubs
Impose civil penalties adopted under ORS 431.415 (1)(c) and enforce the ordinances and rules adopted under ORS 431.415 (1)(b);
Perform any other duty imposed on local public health authorities by law.
Ensure that all core duties under OAR 333-014-0550 are met/adhered to, as identified below:
The following are activities that Oregon law specifically requires a local public health authority to perform:
(a) Accepting reports of reportable disease, disease outbreak or epidemics and investigating reportable diseases, disease outbreaks, or epidemics under ORS 433.004 and 433.006.
(b) Issuing or petitioning for isolation and quarantine orders under ORS 433.121 to 433.142 as necessary to protect the public’s health.
(c) Review of immunization records and issuing exclusion orders under ORS 433.267.
(d) Making immunizations available under ORS 433.269.
(e) Duties and activities related to enforcing the Indoor Clean Air Act under ORS 433.875, if delegated by the Authority.
(f) Ensuring access to family planning and birth control services under ORS 435.205.
(g) Licensure of tourist accommodations, including hostels, picnic parks, recreation parks and organizational camps under ORS 446.310 to 446.350, if delegated by the Authority.
(h) Licensure of pools and spas under ORS 448.005 to 448.100, if delegated by the Authority.
(i)Restaurant licensure, including commissaries, mobile units, vending machines and bed and breakfasts under ORS 624.310 to 624.430, if delegated by the Authority.

Ensure that any required advisory committees are in place and operational.

Be accessible by phone for emergency purposes and provide reasonable support, documentation and follow up in a timely manner if needed.

Serve on various boards or committees in the community as it relates to community services.
-
Research funding sources for public health services. Prepare grant proposals.
-
Ensure that the clinical team is apprised of current evidence-based practices, implement these practices as indicated, and ensure that the agency is meeting or exceeding the standards for good clinical outcomes.

Interview and hire employees. Provide performance evaluations for employees, resolve conflict, follow personnel policies as indicated and provide positive leadership. May be requested to participate in the hiring processes for other CCS personnel.

Lead, coordinate and effectively manage staff, sustaining a high level of collaboration and cooperation. Provide clinical supervision to all clinical staff.

Oversee the activities of all contracted personnel, including professionalism, quality of care, quality of documentation and submission of required information. Work with contracted providers to put together schedules and ensure that the service delivery is well coordinated.

Ensure that the quality of work is superior and completed in a professional manner.

Order supplies and necessary materials for departmental programs.
-
Ensure that programs meet or exceed all of the requirements of the relevant Oregon Administrative Rules (OAR’s), as well as other contractual or grant requirements.

Ensure staff coverage by overseeing vacations, sick leave, training, etc.
-
Serve on multi-disciplinary team and advisory boards relating to health service issues.
-
Schedule and participate in meetings with other agencies to ensure continuity of service delivery and ensure that CCS is highly regarded by community partners. Participate in all internal agency meetings as requested. Hold regular department meetings.

As requested, assist the Executive Director and Chief Financial Officer in the development of, implementation and following of the agency budget.

Follow the grievance process for customer complaints, working diligently to resolve complaints. Respond to requests and direction from the Executive Director.

Provide public health educational presentations to schools and community groups on various health care issues.

Report all cases of abuses or neglect to the correct agency.

Requirements:
QUALIFICATIONS
Education and/or Experience
Bachelor’s degree in public health administration or health business management from an accredited college or university or Equivalent to a four-year degree in public health administration, behavioral, social or health science or related field and five (5) years of nursing/public health experience and education with at least two of those years in a supervisory capacity,
OR
- any combination of education and experience that has provided the knowledge, skills and abilities to perform the essential duties of this position*.
- Prior work experience and educational requirements listed are typical ways of obtaining the required qualifications. Other equivalent combinations of education, training, and experience will be considered.
Other Skills and Abilities
Thorough knowledge of public health and the professional practices of a public health department including laws and regulations governing public health. Ability to plan and organize work, communicate effectively both verbally and in writing and deal tactfully and patiently with the public. Establish and maintain effective and cooperative relationships with the medical community, physicians, related agencies and school officials. Establish and maintain client records and associated medical reports. Must have good typing skills, have knowledge of basic office software programs and ability to effectively use such software. Skill in the use of medical instruments/equipment and assessment tools and/or willingness to learn. Must be able to communicate effectively in both written and oral formats. Must have the ability to present and exchange information internally across teams and co-workers, and externally with customers and the public. Information communicated ranges from routine/basic to complex and confidential information.
CRIMINAL BACKGROUND CHECKS
Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 410-007-0200 through 410-007-0380. In addition to a pre-employment background check, each employee, volunteer and contractor shall be checked on a monthly basis against the OIG and GSA exclusion lists, as well as other federal and state agency lists. If it is discovered that an employee, volunteer or contractor is excluded or sanctioned it will be the cause for immediate termination of employment, volunteering, or the termination of the contract.

PERSONAL AUTO INSURANCE
Must hold a valid driver’s license as well as personal auto insurance for privately owned Vehicles utilized for CCS business such as client service purposes, travel between business offices and the community, to attend required meetings and training. Must show proof of $300,000 or more liability coverage for bodily injury and $100,000 or more in property damage and maintain said level of coverage for the duration of employment at CCS. The employee’s insurance is primary with CCS insurance being secondary. CCS reserves the right to deny any employee the use of a vehicle owned by CCS.
PHYSICAL DEMANDS
While performing the essential duties of this job, the employee is regularly required to use office automation including computer and phone systems that require fine manipulation, grasping, typing, and reaching. Sitting for several hours reviewing reports and other written materials and talking on phone. This position may occasionally require you to lift up to 50 pounds. Incumbents will occasionally have to reach, bend, kneel and squat when adjusting equipment or retrieving supplies.
WORK ENVIRONMENT
Work is performed in a medical clinic office environment, schools, homes, and other settings. Exposure to communicable diseases including blood-borne diseases. May require lifting of medical equipment and /or supplies and standing for prolonged periods. The noise level is usually moderate but occasionally may be exposed to loud noise such as raised voice levels and alarms.
This position is exposed to the everyday risks or discomforts which require normal safety precautions typical of such places as an office (i.e., moving mechanical parts, airborne particles, and electrical shock).
