Job Overview

We are seeking a dedicated and highly skilled Long-Term Care (LTC) Pharmacist to join our dynamic healthcare team. In this vital role, you will oversee medication management and ensure optimal pharmaceutical care for patients in long-term care facilities. Your expertise will support patient safety, medication accuracy, and compliance with healthcare regulations. This position offers an exciting opportunity to make a meaningful impact on patient outcomes through comprehensive pharmaceutical services in a fast-paced, collaborative environment.

Responsibilities

Review and verify medication orders for accuracy, appropriateness, and safety within LTC settings
Collaborate with healthcare providers to develop and implement individualized medication plans
Ensure proper medication storage, handling, and documentation in accordance with regulatory standards
Provide expert guidance on drug interactions, contraindications, and side effects to clinical staff
Conduct patient assessments to evaluate medication effectiveness and identify potential adverse reactions

Requirements

Pharmacist license in good standing with relevant state licensing boards
Strong knowledge of physiology, anatomy, medical terminology, and acute care practices
Excellent communication skills with a focus on patient care and interdisciplinary collaboration
