Intensive Community Based (ICB) Case Manager

Schedule: Primarily weekdays with some evenings; participates in crisis/on-call rotation as required.
Overview
The Intensive Community Based Case Manager provides high‑ intensity, community based‑ services to children and youth typically ages 10-18, who require multi-systemic coordination of services to successfully remain out of the hospital and/or juvenile services, with focus on interventions and the frequency of service needed for the client to remain in the home.
This position works in partnership with the Intensive Community Based Therapist to ensure that each youth’s individualized treatment plan is implemented across systems and settings. The Case Manager delivers services from a strengths-based perspective and as identified in the client individualized treatment plan to teach skill building such as coping strategies to stabilize crises, strengthen family functioning, and prevent or divert from out of home settings or inpatient psychiatric hospitalization. In addition, the ICB Case Manager completes assessments, such as PHQ9, AUDIT or others approved by SRMHW as needed for CCBHC client outcome data.
Primary Duties and Responsibilities
1. CCBHC Access, Engagement & Care Navigation
- Ensure timely contact with referred families in alignment with CCBHC access standards (rapid engagement, same day/next ‑day‑ crisis pathways).
- Provide family friendly orientation to services, program expectations, and available supports‑.
- Assist families in overcoming barriers to care (transportation, scheduling, insurance, basic needs, school coordination).
- Services are to be delivered from a strength-based perspective, including strengths in treatment planning, skill building, and in advocacy for clients.
2. Care Coordination & Team Support
- Work daily in coordination with the Intensive Therapist to implement individualized treatment plans and align goals across settings.
- Coordinate services across multiple systems, including:
- schools
- juvenile justice/probation
- child welfare
- primary care/medical providers
- community organizations and natural supports
- Provide support for the client during Child & Family Team meetings and ensure youth/family voice guides all planning.
- Assist families with referrals for medical, psychiatric, SUD, housing, respite, crisis stabilization, and other support.
3. Level‑of‑Care Monitoring & Intensive In‑Home Support
- Monitor symptoms, functioning, environmental stressors, and service participation to ensure the youth is receiving the correct level of care (LOC).
- Conduct frequent home, school, and community visits proportional to acuity and level of care.
- Support treatment plan goals which may include:
- skill reinforcement building and practice, which include development of strategies to improve behavioral/emotional dysregulation.
- modeling of de‑escalation and positive behavior strategies
- coordination of structured routines and safety plans
- Complete assessments for client outcomes measures as identified by SRMHW to meet CCBHC requirements.
4. Crisis Prevention & Diversion
- Participate in the CCBHC 24/7 on call crisis response system as assigned; assist with risk screening, safety planning, and follow-up‑.
- Provide proactive support aimed at:
- detention diversion
- hospital diversion
- PRTF diversion
- Coordinate and communicate among the ICB Team client discharge plans from inpatient psychiatric hospitalization and other higher levels of care.
- Assist families in implementing safety plans in real time‑ and provide follow-up services with intent and frequency to maintain home and community living.
5. Resource Linkage & Family Support
- Address social determinants of health (SDOH) by connecting families to basic needs: food, housing, financial assistance, and transportation.
- Help caregivers navigate complicated systems, complete applications, gather documentation, and attend appointments.
- Provide psychoeducation on mental health, developmental stages, trauma impacts, school supports (IEPs/504s), and healthy communication skills.
- Strengthen natural supports (extended family, mentors, community groups).
6. Documentation & CCBHC Quality Requirements
- Complete progress/service notes, release forms, referrals, and care coordination records in compliance with CCBHC documentation timelines.
- Track outcomes, engagement, and diversion activities for CCBHC quality measurement standards.
- Uphold HIPAA, 42 CFR Part 2, and all organizational compliance requirements.
Required Qualifications
- Bachelor’s degree in social work, Psychology, Human Services, or related field strongly preferred (or equivalent experience per state regulations).
- Experience working with children, adolescents, and caregivers experiencing high behavioral health needs.
- Knowledge of community resources, case management practices, and cross system‑ collaboration.
- Valid driver’s license, insurance, and reliable transportation.
- Ability to work flexible hours and travel within the service area.
- Ability to successfully complete KBI, SRS child and adult abuse registry checks, alcohol/drug screening process, and motor vehicle record checks.
Preferred Qualifications
- Experience in a CCBHC, wraparound services, intensive in-home‑ programs, crisis services, child welfare, or juvenile justice.
- Training/knowledge of trauma-informed‑ care, restorative practices, parent coaching, or behavior support strategies.
- Bilingual abilities (e.g., English/Spanish).

Typical Physical Demands:
This position requires sitting for long periods of time. It may require contact with individuals with communicable diseases. Requires the ability to work under high stress conditions for long periods of time. Ability to hear normal range of voice, prepare written reports, and to use office equipment and computer software.
Typical Working Conditions:
The ICB Case Manager provides intensive community-based mental health services to children and adolescents who require multi-systemic coordination of services to successfully remain out of a higher level of care, such as the inpatient psychiatric hospitalization and detention, with all treatment strategies focused on these youth remaining safely in the community. ICB staff will work as a multidisciplinary team in providing services as needed in the community and client home environments. ICB staff will include parents or guardians as integral to the treatment planning process, providing support, community referrals, mental health education, and required to include families and other significant people in information sharing and information gathering as needed. Individuals may expect occasional exposure to communicable disease.
