Full Job Description

Full-Time Riverside/San Bernardino Social Worker

RESPONSIBILITIES:

The Social Worker is responsible for the delivery of comprehensive trauma-informed, culturally relevant, age and developmentally appropriate services to placed children/youth/NMD to ensure their safety, well-being, and permanency. The Social Worker ensures coordination of care while collaborating with the youth, family, and Child and Family Team, to promote successful service and permanency planning and meet goals and objectives as per the treatment plan. The Social Worker will carry a maximum caseload as per state regulations and/or program requirements.

DUTIES:

1. Participates in the pre-placement screening, completes intake referral log and intake screening form, coordinates placements, assists in the initial placement of children/youth/NMD in resource homes, and provides orientation to newly placed child/youth/NMD.

2. Conducts visits with resource parents and children/youth/NMD to monitor the placement of children in resource homes.

3. Provides appropriate referrals for clients to assist them in meeting their treatment goals (i.e. tutoring, vocational training, mental health services, regional center)

4. Collaborates with Child and Family Team to develop service plans; writes needs and service plans/quarterly reports, and monitors the progress of treatment goals.

5. Participates in CFT and team meetings (i.e. IEP, SST, MAT summary of findings, Wraparound meetings, etc.) with child/youth/NMD, family, support individuals, treatment staff, education providers, placing agency social work staff.

6. Meets with resource parents, children, and birth parents and/or birth relatives as required by the treatment plans; supervises and/or arranges and maintains documentation of visits between children and birth parents/relatives.

7. Maintains documentation in case files of home visits, significant telephone contacts, and all required documentation as required by CCL, DCFS, and FFA.

8. Ensures health and safety standards are being met by monitoring medical/dental, food, and clothing needs of children/youth, as well as their home environment.

9. Reports any substantiated or suspected child abuse allegations to the appropriate persons and agencies.

10. Composes required reports (initial, quarterly and discharge reports) and submits to placing agency within required timeframes; composes special incident reports and submits to CCL and placing agency within required timeframes.

11. Ensures the resource home meets Title 22, DCFS, and Agency health and safety standards. Conducts home inspections of resource homes, documents violations and/or deficiencies of regulations and reports said violations to administration; works with resource parents to correct deficiencies and monitors on-going compliance with required home safety standards; provides at minimum monthly documentation regarding overview of resource family/home; completes reevaluation reports and updates as needed, participates in quality assurance and utilization review actions, participates in Administrative Review meetings.

12. Participates in the recruitment of foster parents, conducts orientations and preapproval trainings of prospective resource parents; conducts home studies and/or prepares written home assessment reports with recommendations for approval; conducts home inspections of prospective resource homes.

13. Acts as a liaison with placement agency workers and other community resource organizations.

14. Provides on-call emergency support to resource families and children.

15. Attends required supervision, staff meetings, and trainings.

16. Participates in Peer Reviews.

17. Participates in Agency/Program performance and quality improvement projects.

18. Adheres to agency safety protocols.

QUALIFICATIONS:

1. Masters Degree from an accredited or state approved graduate school in social work or social welfare, marriage, family, and child counseling, child development, counseling psychology or social psychology.

2. Minimum of one (1) year work experience as a social worker in an agency serving children.

3. Demonstrates ability to work as part of a multi-disciplinary team

4. Valid California’s Drivers License and insurable driving record

5. Criminal record and health clearances

6. Demonstrated experience working with an ethnically, culturally and racially diverse client populations

PHYSICAL SKILLS AND ABILITIES:

1. Must have the visual, physical and technical ability to independently operate a variety of office equipment including the use of a computer.

2. Must be capable of bending, stooping, filing, lifting of at least 25 lbs. and able to interrupt physical altercations.

3. Must have the ability to verbally communicate well with others.

ORGANIZATION RELATIONSHIPS/INTERACTIONS:

1. Interfaces with all levels of management, staff, clients and families including partner agencies
