This Case Management Nurse Director opportunity in Western Montana offers an exciting chance to step into a newly opened leadership position at a highly respected acute care hospital. This role is perfect for an experienced RN leader who is passionate about improving patient outcomes, strengthening care coordination, and supporting a collaborative healthcare team. The Case Management Nurse Director reports directly to the Chief Nursing Officer and will work closely with physicians, nursing leaders, social services, and interdisciplinary teams throughout the hospital. As a newly opened leadership position, the Case Management Nurse Director will have the opportunity to help guide care coordination efforts while building strong relationships across multiple departments.

The Case Management Nurse Director will oversee hospital case management services focused on safe patient transitions, discharge planning, utilization management, and continuity of care. This role partners with experienced clinicians across the organization to ensure patients receive coordinated, high-quality care from admission through discharge.
