Full Job Description

Mountain View Health and Rehabilitation is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our clinical team for both day and night shifts. Our nurses play a vital role in providing high-quality, resident-centered care in a skilled nursing environment. Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with physician orders, care plans, facility policies, and state and federal regulations. The LPN works collaboratively with Registered Nurses, Certified Nursing Assistants, providers, and the interdisciplinary team to ensure safe, effective, and compassionate care across all shifts.

Essential Duties and Responsibilities

Clinical Care

Provide direct nursing care including medication administration, treatments, and skilled nursing interventions as ordered.
Monitor residents for changes in condition and promptly report findings to the RN or provider.
Perform focused assessments and contribute to care planning and evaluations.
Accurately document all care, assessments, and observations in the medical record.
Respond appropriately to emergencies and changes in condition, including participation in CODE responses when applicable.

Medication & Treatment Management

Administer medications and treatments safely and in accordance with the “Five Rights” of medication administration.
Monitor for adverse reactions or side effects and report concerns promptly.
Ensure proper storage, handling, and documentation of medications.

Resident Safety & Quality of Care

Implement and follow fall prevention, infection control, and safety protocols.
Ensure residents’ rights, dignity, and privacy are respected at all times.
Support skin integrity, hydration, nutrition, and comfort measures.

Team Leadership & Communication

Provide direction and support to CNAs to ensure quality resident care.
Communicate effectively with residents, families, and interdisciplinary team members.
Participate in shift reports, care conferences, and quality improvement activities.

Compliance & Professional Practice

Adhere to all facility policies and procedures as well as state and federal regulations.
Maintain current licensure and required certifications.
Support survey readiness and participate in audits or corrective actions as needed.

Day vs. Night Shift ExpectationsDay Shift

Participate in provider rounds, care conferences, and interdisciplinary collaboration.
Communicate with families and support services as needed.
Assist with admissions, discharges, and ongoing care coordination.

Night Shift

Provide vigilant monitoring of residents and respond promptly to changes in condition.
Complete scheduled assessments, treatments, and documentation.
Ensure a calm, safe, and supportive overnight environment for residents.

QualificationsRequired

Current Licensed Practical Nurse (LPN) license in good standing in the State of Montana.
Ability to work day or night shifts as scheduled.
Strong clinical skills and attention to detail.
Ability to work effectively as part of a team.

Preferred

Experience in skilled nursing or long-term care.
Familiarity with electronic medical records.
Strong communication and leadership skills.

Physical & Work Requirements

Ability to stand, walk, bend, lift, and assist residents as needed.
Ability to respond quickly in emergency situations.
Work environment includes resident rooms, nursing stations, and common areas.
