Peer Mentor - Medical

Reports To:

Vice President of Medical & Strategic Partnerships

Required Qualifications:

High school diploma or GED
1 year minimum demonstrated personal recovery experience
KCPM Certification or willingness to obtain
Current valid driver’s license

Preferred Qualifications:

1 year experience in SUD treatment facility

Knowledge, Skills, Abilities:

Excellent interpersonal and customer service skills.
Excellent organizational skills and attention to detail.
Ability to function well in a high-paced and at time stressful environment.
Proficient with Microsoft Office Suite.
Effective oral and written communication skills.
Ability to interact effectively and professionally.
Ability to prioritize tasks and delegate when appropriate.
Ability to always maintain confidentiality and professionalism.

Position Summary:

This position provides direct clinical services to patients presenting and/or admitted to a medical facility for acute alcohol/drug intoxication. Sharing their own lived experience of recovery from substance use, they serve as a mentor, advocate, and motivator for patients working toward achieving long-term recovery. The Peer Mentor works as a member of the addiction health care team, engaging patients in recovery planning, treatment services, and follow-up care.

Essential Job Functions:

Complete clinical screening, evaluation, placement of patients, and discharge/transition plans.
Coordinate clinical services to minimize hospital utilization and maximize treatment engagement.
Collaborate with hospital staff to improve management of patients.
Coordinate Substance Use Disorder services provided to Emergency Services Department and additional areas of hospital as needed.
Assist with planning and implementing clinical strategies for the patients stay in acute care settings.
Plan and implement clinical strategies and resources to achieve increased engagement of patients in treatment services.
Identify and coordinate the care planning for high utilizers of acute services and develop interventions to encourage utilization from Emergency Services to community-based services.
Interview patients, review records, and confer with other professionals to evaluate condition of patients.
Counsel patients to assist them in overcoming alcohol and drug dependency.
Counsel family members on how to cope and provide support for the patient, the impact dependency has on their lives and counsel when in crisis as needed.
Refer patients to appropriate services as needed such as detoxification, medical evaluation and treatment, social services, and recovery resources.
Maintain contact on a regular basis with referral sources.
Monitor conditions of patients to evaluate success of therapy and adapt treatment as needed.
Maintain patient files and keep prepared for site visits.
Understand and implement all TAP 21 Addiction Counseling Competencies.
Participate in utilization review activities as required.
Comply with all CKF policies and procedures and related clinical licensure standards.
Other duties as assigned.

Based on an 8-hour day

Sit – Continuously – 5.5 – 8 hrs daily

Stand - Continuously – 5.5 – 8 hrs daily

Lift/Carry 0-10 lbs. – Occasionally - .25 – 2.5 hrs daily

Walk – Frequently – 2.5 – 5.5 hrs daily

Reach Above - Occasionally - .25 – 2.5 hrs daily

Bend/Stoop - Occasionally - .25 – 2.5 hrs daily

Use of hands – Pushing/Pulling - Occasionally - .25 – 2.5 hrs daily

Use of hands – Fine Manipulation - Continuously – 5.5 – 8 hrs daily

PHI Access Level:

Level 2 – This position will have regular access to patient records, no access to employee medical information.

Clinical environment with moderate noise level.
