JOB SUMMARY:
The Director of Health and Wellness provides strategic and operational leadership for resident care within the community, managing, directing, and monitoring nurses, resident care team members, and medication aides as applicable by state law. This role is responsible for promoting the health, safety, and well-being of the resident population in an assisted living and/or memory care setting. The Director of Health and Wellness must have a genuine desire to work with seniors, demonstrate tolerance and compassion for behaviors associated with dementia and other aging processes, and uphold CIMA’s core values in all aspects of care and leadership.
QUALIFICATIONS:
- College degree required; graduate of an accredited LVN/LPN/RN program preferred.
- 2–3 years of related experience, preferably in an assisted living or memory care setting; experience in geriatric nursing or emergency/first aid care preferred.
- If licensed, must possess a current, unencumbered, active license to practice as an RN or LVN/LPN in this state; must maintain valid continuing education credits as required by state.
- Current CPR certification required.
- Valid driver’s license required.
- Computer proficiency with electronic medical records (EMR), the Microsoft Office suite, and the ability to learn new applications.
- Demonstrated written communication proficiency at a 12th grade to college level; the role requires the ability to produce formal written reports, regulatory correspondence, and sensitive investigative documentation — including incident narratives, abuse and neglect allegations, investigation findings, resident care documentation with evidentiary weight, and mandatory reports to governing authorities — with accuracy, clarity, and a professional register appropriate to their legal and clinical context. Excellent verbal communication and customer service skills are also required.
- Must be flexible in hours, shifts, and days available to work, including evenings and weekends.
SPECIALIZED KNOWLEDGE AND ABILITIES:
- Strong clinical knowledge of assisted living and memory care processes, including resident assessment, individualized service planning, and care delivery.
- In-depth understanding of applicable federal, state, and board of nursing regulations governing assisted living and memory care communities.
- Demonstrated ability to lead, mentor, and hold clinical care teams accountable while fostering a supportive and high-performing work environment.
- Skilled in identifying, tracking, and trending clinical quality data to proactively mitigate risk and improve resident outcomes.
- Exceptional written and verbal communication skills; ability to produce clear, professionally precise documentation — including incident narratives, investigation findings, abuse and neglect reports, state-required clinical records, and regulatory correspondence — and to communicate effectively with residents, families, physicians, pharmacists, interdisciplinary team members, regulatory agencies, and executive leadership.
- Strong customer service orientation with the ability to investigate and resolve resident and family concerns with empathy and urgency.
- Experience with medication management programs and oversight of safe medication use processes.
- Ability to perform skill evaluation assessments and ensure ongoing clinical competency of care team members.
- Organized and detail-oriented; able to maintain accurate resident charts, state-required documentation, and regulatory readiness at all times.

ESSENTIAL JOB FUNCTIONS AND TASKS
Clinical Care & Resident Services
- Provides clinical care through the direct application of assisted living and memory care processes; performs and documents resident assessments, completes Individualized Service Plans (ISPs), and provides or delegates hands-on clinical care as indicated by the plan of care.
- Performs timely assessments of potential and current residents to ensure their needs can be, or continue to be, met in an assisted living and/or memory care setting.
- Gathers medical and contact information for all new residents; reviews resident chart documentation for completeness and accuracy.
- Collaborates with physicians, pharmacists, and other clinical providers to coordinate care and services for community residents.
- Provides oversight of the community medication management program to promote resident safety in the medication use process.
- Tracks, trends, and reports clinical quality data to identify and mitigate risk.
- Conducts incident report follow-up investigations and ensures completion of resident status documentation.
- Participates in clinical quality and process improvement initiatives to reduce risk and improve resident care outcomes.
- Maintains accurate, complete, and timely resident charts and all state-required documentation; ensures records meet applicable legal and evidentiary standards and that the community is prepared for state surveys and health inspections at all times.
- Ensures compliance with all applicable federal, state, board of nursing, and other regulatory requirements.
- Communicates directly with all disciplines involved in resident care on a regular basis, including evenings and weekends as needed.
- Serves as a member of the community’s emergency response team.
Team Leadership & Operations
- Recruits, hires, trains, and manages clinical team members; is responsible for performance management, evaluation, and team engagement.
- Schedules clinical and care staff, ensures managers are familiar with and adhere to state regulatory standards, and fosters a supportive, high-morale work environment.
- Performs skill evaluation assessments to ensure clinical competency of care team members; conducts daily rounds to review care staff performance.
- Collaborates with sales partners to assess community capability and evaluate potential new residents.
- Assists with resident transfers and move-outs.
- Responds to concerns of residents, families, and visitors by providing immediate and courteous assistance; investigates and resolves complaints.
- Provides superior customer service in support of resident retention.
General Responsibilities
- Responsible for the overall safety of residents and the community.
- Operates and maintains assigned equipment, supplies, inventory, and other property according to established procedures.
- Maintains a professional appearance and good personal hygiene per company policies.
- Completes all assigned duties, which may change from time to time according to resident needs, staffing levels, and operational circumstances.
- Assists with special projects and other duties as assigned.
Work Environment:
Work is performed in an assisted living and/or memory care community setting, including resident living areas, common spaces, offices, and outdoor areas as needed. The Director of Health and Wellness is regularly required to stand, walk, bend, reach, and lift in the course of providing and overseeing hands-on clinical care. Occasional lifting of up to 50 pounds may be required. The role involves frequent interaction with residents, families, staff, physicians, and visitors throughout the day. There is potential for exposure to blood-borne pathogens, infectious disease, and hazardous materials inherent in a health care environment. Availability to work evenings, weekends, and on-call as needed is required to meet resident and operational needs.
Pay: $75,000.00 - $85,000.00 per year
