Community Health Worker Job Description

Revised Date: April 9, 2026
Reports To: Manager (determined by COO according to programmatic work)
FLSA Status: Non-Exempt
Scope of Authority: This position does not supervise other personnel.
Security/Access: Employee will be issued picture identification and department credit card.

Job Summary

The Community Health Worker (CHW) serves as a liaison between community members, healthcare providers, and social service systems to improve access to care and health outcomes throughout the Two Rivers Public Health Department district. This position provides outreach, education, care coordination, referral navigation, and health coaching to individuals and families while supporting community partnerships, outreach initiatives, and public health programming.

Required Education and Experience

· High school diploma or equivalent required.

· Associate’s or Bachelor’s degree in public health, health sciences, human services, social work, or closely related field preferred.

· Minimum of two (2) years of experience in health, human services, community outreach, or related field preferred.

· Completion of National Incident Management System (NIMS) training within six (6) months of employment.

· Completion of Nebraska DHHS-approved Community Health Worker/Health Navigator certification within one (1) year of employment or alternative approved certificate as determined by the COO.

· Proficiency in Microsoft Office and common computer/database applications.

· Valid driver’s license, proof of personal vehicle insurance, and ability to use personal transportation for work purposes.

· Ability to travel throughout the TRPHD seven-county service area and occasionally beyond.

· Demonstrated ability to work effectively with diverse populations and maintain cultural awareness and sensitivity.

Duties and Responsibilities Community Outreach and Client Support

· Connect individuals and families with health programs, healthcare providers, and community resources.

· Provide education and support related to screening results, medication adherence, exercise and nutrition, healthcare navigation, substance use, and other health-related behaviors.

· Motivate and educate clients regarding the importance of preventive care and follow-up appointments.

· Assist clients in scheduling appointments and navigating healthcare and social service systems.

· Coordinate care with healthcare providers and partner organizations to ensure continuity of services.

· Serve as a health referral resource for community members and partner organizations.

Community Partnerships and Program Development

· Establish and maintain partnerships with community stakeholders, providers, and organizations.

· Support development and maintenance of community coalitions involving Community Health Workers and related partners.

· Assist with planning, preparation, and implementation of outreach events, workshops, and educational activities.

· Represent TRPHD professionally in community settings and collaborative initiatives.

Documentation and Reporting

· Maintain accurate documentation of client interactions, referrals, and program outcomes.

· Collect, maintain, and report information regarding community resources and service availability.

· Prepare reports and support data collection for program evaluation, grant requirements, and organizational initiatives.

Administrative and Organizational Support

· Provide general administrative support for assigned programs and department operations as needed.

· Process documents, maintain filing systems, and ensure security of confidential records.

· Assist with development of messaging and content for outreach and social media efforts.

· Demonstrate sound fiscal accountability in use of departmental resources.

Organizational Participation and Compliance

· Comply with all TRPHD policies, procedures, and confidentiality requirements, including HIPAA regulations.

· Participate in assigned staff committees, required trainings, performance management, quality improvement activities, and accreditation efforts.

· Assist with emergency preparedness and response efforts during disease outbreaks, disasters, or public health emergencies as assigned.

· Complete other duties as assigned.

Physical Requirements

· Ability to sit, stand, walk, drive, and use hands/fingers for routine office and outreach activities.

· Ability to occasionally climb stairs, stoop, kneel, crouch, or crawl.

· Ability to lift and/or move up to 50 pounds.

· Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.

· Ability to participate in and demonstrate health education and wellness activities as required by assigned programs.

Work Environment

· Ability to work in a smoke/tobacco-free and drug-free work environment.

· Ability to work occasional evenings and weekends based on programmatic or community needs.
