Full Job Description

Job Summary
Solterra Hospice is looking for a team-oriented, compassionate RN to join our amazing interdisciplinary team. Responsibilities

Comprehensive assessments and documentation.
Teaching patients, families, and caregivers about medications used in symptom management.
Wound assessment and wound care management.
Educate patients and families on disease progression, medication use, advanced directives, and comfort measures to promote informed decision-making.
Coordinate interdisciplinary care by collaborating with physicians, social workers, therapists, and other healthcare professionals to optimize patient outcomes.
Document all patient interactions accurately using EMR (Electronic Medical Records) systems while adhering to HIPAA regulations.
Respond promptly to calls and urgent situations as they arise.
Maintain a positive, team-oriented culture within our organization.

Skills

Must be able to maintain excellent communication and positive interactions, including with co-workers, patients, families, facility partners, etc.
Proven experience in hospice & palliative care with strong knowledge of pain management and symptom control strategies.
Experience working with diverse populations including geriatrics, patients with developmental disabilities, memory care needs, or those in assisted living or long-term care settings.
Strong understanding of infection control practices and aseptic techniques essential for safe patient care.
Ability to perform physical examinations, vital signs assessment, medication reconciliation, and diagnostic evaluation in home or community environments.
Excellent communication skills for teaching families about disease processes and providing emotional support during difficult times.
Certification in Basic Life Support (BLS) required.

Pay: $49.00 - $54.00 per hour
