Destination Hope & Recovery is looking for a compassionate, motivated, and community focused individual to join our team as a Choice Case Manager. Our Choice Program provides community based care coordination, resource navigation, referrals, advocacy, and ongoing support to individuals facing complex life challenges. What You'll Do
As a Choice Case Manager, you will build strong, trusting relationships with clients while helping them identify goals, overcome barriers, and navigate complex systems of care. Responsibilities include:
- Complete comprehensive assessments to identify client strengths, needs, and priorities.
- Develop individualized service plans that reflect each client's goals and support long term success.
- Provide ongoing case management, care coordination, advocacy, and follow up services.
- Connect clients with housing resources, healthcare providers, behavioral health services, employment programs, transportation, financial assistance, and other community resources.
- Advocate for clients while helping them access services and navigate multiple systems.
- Provide crisis intervention and support individuals experiencing homelessness, behavioral health concerns, substance use disorders, or other urgent challenges.
- Coordinate with landlords, property managers, housing providers, and community partners to support housing stability while maintaining compliance with Fair Housing regulations.
- Deliver culturally responsive and trauma informed services while working effectively with diverse populations and underserved communities.
- Maintain accurate, timely, and detailed electronic case records, documentation, and reporting requirements.
- Utilize technology and electronic health record systems to document services, manage caseloads, and communicate with providers and community partners.
- Collaborate with internal staff and external agencies to ensure coordinated, client centered care.
- Participate in team meetings, trainings, and professional development opportunities.

- Is comfortable using computers, electronic documentation systems, Microsoft Office, and other technology.
- Understands trauma informed care, harm reduction principles, and person centered service delivery.
- Values diversity, equity, and inclusion and is committed to serving individuals from all backgrounds with dignity and respect.
Preferred Qualifications
- Experience in case management, behavioral health, social services, housing services, peer support, or human services.
- Knowledge of community resources throughout Cowlitz and Wahkiakum Counties.
- Experience working with individuals experiencing homelessness, mental health conditions, substance use disorders, or other complex needs.
- Lived experience in recovery or navigating behavioral health systems is valued and encouraged.

Pay: $23.00 - $26.00 per hour
