Full Job Description

SUMMARY

The Permanent Supportive Housing (PSH) Case Manager skillfully provides strength-based, trauma-informed support to families experiencing chronic homelessness and housing instability. The PSH Case Manager is responsible for the implementation of individualized service plans, coordination of supportive services, and engagement with residents that promotes long-term housing stability and improved social determinants of health. This role integrates the principles of harm reduction, housing first, and motivational interviewing to address complex needs such as mental health challenges, substance use, domestic violence, developmental disabilities, and systemic barriers to housing. This role ensures compliance with grant requirements, agency protocols, and best practices in service delivery across the housing continuum.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Support Resident Stability & Well-Being

Conduct intake assessments and develop individualized service plans focused on life skills, housing preservation, economic stability, and family well-being.
Provide consistent, high-quality case management through regular visits and communication that is tailored to resident needs.
Utilize strength-based, hospitable, and trauma-informed approaches to build positive relationships with residents.
Help participants overcome psychological, emotional, and social barriers by connecting them to appropriate services and support.
Support goal setting in areas such as financial wellness, parenting, healthcare access, education, and employment.
Aid in the preparation and submission of important public benefits (e.g., social security, MI Bridges, MSHDA recertification).
Assist with crisis resolution, housing preservation, and conflict mediation between residents, landlords and/or service providers.
Advocate on behalf of underserved or oppressed residents and support the development of resident-led solutions.

Community & Household Engagement

Facilitate group and individual programming that fosters leadership, connection, and empowerment within the resident community.
Plan and implement resident events such as council meetings, educational sessions, cultural celebrations, and youth programs.
Promote resident participation in property management, community-building initiatives, and safety programming.
Collaborate with the Resident Engagement team to identify potential partners and assist in coordinating workshops or events

Collaboration & Program Development

Partner with internal teams and external stakeholders to ensure wraparound services are coordinated and accessible.
Support continuous quality improvement by participating in program evaluation and feedback loops.
Assist with outreach and marketing of ICCF programs and community engagement efforts.
Develop familiarity with community resources and maintain updated information for referrals.

Documentation, Compliance & Grant Support

Maintain accurate, timely documentation in HMIS and other data systems to meet grant and programmatic reporting standards.
Contribute to grant applications, renewals, and reporting by tracking metrics and success indicators.
Ensure all service delivery complies with federal, state, and local guidelines, including HUD PSH regulations.

QUALIFICATIONS

Bachelor’s degree in Social Work or related field preferred;
Minimum of 3–5 years of experience in case management or supportive housing, preferably with chronically homeless or highly vulnerable populations.
Knowledge and experience in housing first practices, trauma-informed care, motivational interviewing, and harm reduction.
Demonstrated ability to work effectively across cultures and a commitment to racial equity and justice.
Strong interpersonal, organizational, and communication skills.
Experience with HMIS or other client management systems preferred.
Bilingual in Spanish preferred.
Valid Michigan driver’s license and reliable transportation required.

WORKING CONDITIONS

Full-time (40–45 hours/week); evening or weekend hours may occasionally be required.
This role includes both office work and travel to client homes and community sites.
