Job Description:
Summary:
Responsible for the daily operations and workflow of the entire case management program including care coordination, social work, resource management, and support services. Responsible for developing and maintaining work schedules, orienting new staff members and providing education, mentoring, and support to staff. Actively participates in quality improvement activities through collection of data, analysis, and development & monitoring of action plans to improve key indicators of performance. Analyzes workload and personnel needs and makes recommendations for increase/decrease to the Director. Plans, delegates, and controls work assignments. Evaluates performance of subordinates. Handles complaints, grievances, and employee discipline in collaboration with the Director. Implements policies, procedures and monitors compliance within scope of practice. Demonstrates sensitivity and understanding of the diverse cultural background of patients and caregivers. Possesses excellent interpersonal communication, judgment and problem solving skills. Is able to take initiative, demonstrate follow through and be able to work independently. Participates in the budgetary process and is responsible for proper utilization of labor and operational expenses in accordance with budget. Possesses strong knowledge of disease processes, appropriate clinical interventions and the care of high-risk populations of all age groups. Remains current with state, federal, and provider mandates as they relate to case management. Works collaboratively with medical staff, nursing, and multidisciplinary care team. Shadows staff at regular intervals (including unit rounds) ensuring case management staff proactively engage healthcare team and interventions reflect patient advocacy. Monitors documentation by case management staff to ensure expectations are being met.

Requirements:
Bachelor's degree(BSN) preferred or Master's Degrees Social Worker 2 -3 years in a clinical leadership position and/or 3 - 5 years case management experience.

Licenses and Certifications:
RN or LMSW/LCSW.
