Job Summary
The expectation of this position is assuming responsibility for the overall operation and quality of the group home program; responsible for providing care, supervision, training, and opportunities for growth to individuals at the group home; including assistance with acquisition, retention, or improvement in therapeutic skills needed to enhance the quality of life of the adolescents being supported. Age appropriate therapeutic training and instructions are coupled with elements of support, supervision andengaging participation to reflect the natural flow of training, practice of skills, and other activities as they occur during thecourse of the person's day. Interactions with the individuals served are designed to achieve outcomes identified on the personcentered plan. Based on the individual served and the goals in their person centered plan, the level of needs may be morehabilitation or may be personal care.
This position is under the direct supervision of a Qualified Professional.
Duties
- Supervise in home Direct Support staff
- Make sure the assigned duties of staff are completed
- Oversee resident’s schedules and weekly appointments
- Ensure the needs of residents are being met, in accordance to their PCP
- Coordinate trainings with Coordinator for staff to ensure their certifications are current
- Ensure maintenance of the home is completed in accordance with the schedule
- Maintain contact with residents’ families
- Responsible for the management of residents’ medications (e.g. maintain supply, supervise passing of medications, and documentation)
- Participate in required training
- Maintain required records
- Adhere to Trillium, Alliance, Partners, Vaya, Healthy Blue, NCDHHS, and relevant agency rules and regulations
- Follow Company policy and procedures
- On call 24 hours a day, 7 days a week
- Perform other duties as directed/required
Experience
- Prior experience working with children or adolescents in mental health or behavioral health settings is highly desirable
- Demonstrated knowledge of crisis management protocols and crisis intervention methods
- Hands-on experience with home & community care services or hospital-based patient care environments is preferred
- Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards is a plus
- Strong communication skills for conducting intake interviews, patient assessments, and child & family counseling sessions
POSITION REQUIREMENTS:
1. Proof of Degree, and the ability to read and write proficiently in English. MUST HAVE A DEGREE
2. Reliable transportation.
3. Current state driver's license (or state ID if not driving consumers).
4. Current automobile insurance and registration.
5. Provide at least 2 references of previous work experience.
6. Current CPR card. (If applicable)
7. Current First Aid certification. (If applicable)
8. Medication Administration Certification (if applicable)
9. Proof of mandated state trainings in accordance with requirements of NCDHHS.
10. Clear criminal background check, acceptable Criminal background record as defined by J4 Stability and Safe Haven Policy and Procedures.
11. Health Care registry check, sex offender registry check and other mandated state registries as defined by J4 Stability and Safe Haven Policy and Procedures
12. Acceptable motor vehicle driving record as defined by J4 Stability and Safe Haven Policy and Procedures.
WORKING CONDITIONS:
- Majority of work will be in a home environment,
- May be exposed to stressful situations
- Automobile for transporting residents
- Work days are subject to fluctuate and you will be expected to be available up to 40hrs per week, Monday through Sunday
The following credentials and qualifications apply to staff described in this Subchapter:
(1) "Associate Professional (AP)" within the mental health, developmental disabilities and substance
abuse services (mh/dd/sas) system of care means an individual who is a:
(a) graduate of a college or university with a masters degree in a human service field with less
than one year of full-time, post-graduate degree accumulated mh/dd/sa experience with the
population served, or a substance abuse professional with less than one year of full-time,
post-graduate degree accumulated supervised experience in alcoholism and drug abuse
counseling. Supervision shall be provided by a qualified professional with the population
served until the individual meets one year of experience. The supervisor and the employee
shall develop an individualized supervision plan upon hiring. The parties shall review the
plan annually; or
(b) graduate of a college or university with a bachelor's degree in a human service field with less
than two years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the
population served, or a substance abuse professional with less than two years of full-time,
post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse
counseling. Supervision shall be provided by a qualified professional with the population
served until the individual meets two years of experience. The supervisor and the employee
shall develop an individualized supervision plan upon hiring. The parties shall review the
plan annually; or
(c) graduate of a college or university with a bachelor's degree in a field other than human
services with less than four years of full-time, post-bachelor's degree accumulated mh/dd/sa
experience with the population served, or a substance abuse professional with less than four
years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism
and drug abuse counseling. Supervision shall be provided by a qualified professional with
the population served until the individual meets four years of experience. The supervisor and
the employee shall develop an individualized supervision plan upon hiring. The parties shall
review the plan annually; or
(d) registered nurse who is licensed to practice in the State of North Carolina by the North
Carolina Board of Nursing with less than four years of full-time accumulated experience in
mh/dd/sa with the population served. Supervision shall be provided by a qualified
professional with the population served until the individual meets four years of experience.
The supervisor and the employee shall develop an individualized supervision plan upon
hiring. The parties shall review the plan annually.
