Location: Boise, ID (Hybrid/Remote Available)
Pay: $22-$24.50 per billable hour (DOE)

Hours: 30-40 hours per work

We’re looking for a case management provider with a Bachelor’s degree in a human service field of study who is passionate about providing community-based care in innovative and person-centered ways at a clinic that is based in advocacy and compassion. We’re seeking someone who wants to be seen, supported, and heard as a real human in their workplace.

Role Responsibilities

Duties:

Conduct assessments to identify client needs and develop personalized service plans
Coordinate services across various programs, including but not limited to childcare, education, emergency services and support and workforce development
Provide support, guidance, and advocacy
Provide crisis intervention and management to clients facing urgent situations
Maintain accurate records of client interactions, service plans, and progress notes
Collaborate with a variety of community resources
Work as a team to ensure the best client care possible

What We’re Looking For

Required:

Bachelor’s degree in a human service field of study

Strong communication and a collaborative mindset

Nice to Have:

Experience (or strong interest) working with kids and families
CANS certification (training provided to obtain certification if needed)
Interest in social justice, advocacy, education rights, and access to medical care
