Full Job Description

Function: Under the direction of the Director and Medical Director, the Social Worker is responsible for overseeing the Social Services and needs of patients, family, and staff of the assigned dialysis facility(ies) including the home program. The Social Worker helps patients understand their rights and responsibilities, guide them in managing the physical, mental, emotional and financial needs of their disease. Social Worker actively participates in Quality and Performance Improvement activities in order to meet goals. Duties and Responsibilities: Other duties may be added.

Participates in facility-sponsored social service educational activities with the staff and members of community.
Refers possible quality of care, risk management, or utilization management problems promptly and appropriately. Communicates with superiors regarding situations that differ from the usual.
Evaluates the effectiveness of the patient resources program on an ongoing basis, identifies opportunities for improvement and implements processes to improve the program.
Develops systems to ensure timely and efficient identification of dialysis patients who have social service needs.
Ensures that an assessment and reassessment, as indicated, of psychological, social, environmental, and financial impact of illness on all patient referrals and on those identified as having a potential need is conducted in a timely manner (30 days for assessment and 90 days for reassessment) to determine the patient’s and family’s capabilities in relation to the patient’s anticipated needs.
Consistently looks beyond the identified reason for referral to assess physical or emotional needs that may impact planning.
Contributes to the multidisciplinary plan of care as indicated according to the needs of the patient/family or healthcare team.
Ensures that visits, including problem identification, plan of action and resolution are documented appropriately and in a timely manner (as prescribed by policy).
Ensures that initial visits with new patients are conducted in a timely manner (as prescribed by policy).
Ensures that follow-up visits are conducted in a timely manner (as prescribed by policy).
Documents all patient/family education provided on the Medical Record.
Ensures that social service attends care conferences as indicated to provide social information/input to the primary nurse and the health care team.
Demonstrates the ability to effectively assist patients with pharmaceutical subsidies as indicated
Ensures that all regulatory requirements for social services in the facility are met.
Acts as a resource to members of the healthcare team in meeting the social work needs
Ensures that patients at high risk for suicide are identified and appropriate actions are implemented.
Ensures that patients at risk for being physically or mentally abused and/or neglected are identified and appropriate actions taken.
Ensures that assistance is provided as needed to dialysis support groups.
Ensures that goal-oriented and time measured interventions are directed toward management per identified needs.
Assists with strengthening the family support system.
Assists with resolution of conflict related to the chronicity of illness.
Addresses external discharge obstacles. Oversees referrals to Home Care, Short-Term Rehabilitation, Long-Term Placement, Terminal Care Facilities, etc.
As assigned by supervisor. Competencies ADULTS:
Able to recognize indicators of domestic violence, abuse, abuse, neglect, and sexual assault and makes appropriate referrals.
Demonstrates awareness of and adherence to abuse and protective services laws.
Effectively demonstrates ability to counsel the patient and family members regarding issues of death and dying.
Demonstrates and applies knowledge of human development, of the potential for human growth, of social systems, and of the availability of social resources. Demonstrates knowledge of the principles of growth and development over the life span to assess age-specific needs of the patient and provide for the appropriate care.
Knowledgeable of emotional, cognitive, educational, social and daily activity needs.
Explores the impact of illness and hospitalization to work/job/career, family, and body image.
Watches for body language as a cue for the patient's feelings.
Assesses level of understanding and ability to learn. Provides essential teaching in a manner in which the individual learns best.
Identifies ethical structure.
Applies principles of adult learning when instructing adult patients. Includes patient in decision making and involves significant others as appropriate.
Familiar with the special needs of the adult patient and can access appropriate resources. GERIATRICS: additionally:
Knowledgeable of the aging process and emotional, cognitive, educational, social and daily activity needs. • Explores the patient's support system.
Able to assess the abilities and limitations of the patient.
Assesses level of understanding and ability to learn. Provides essential teaching in a manner in which the individual learns best.
Demonstrates sensitivity to sensory/cognitive needs of the geriatric patient.
Includes the patient in decision making and involves significant others as appropriate.
Familiar with the special needs of the geriatric patient and can access appropriate resources.

Qualifications:

Required: Licensed as Clinical Social Worker in the State of FL .
Master’s degree in Social Work (MSW) required with a specialization in clinical practice.
Two years of experience as a social worker in a healthcare setting or care of critically ill patients required, at least One year of dialysis/Renal experience preferred.
Demonstrated knowledge of government and private insurance programs required.
Bilingual English/Spanish preferred.
Efficient time management skills, results oriented, and able to multi-task
