Full Job Description

Organizational Overview:

Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to promoting health, empowerment, and safety for communities affected by drug use and poverty. PPP meets drug users where they are, providing information and access to resources with dignity and respect. Using a harm reduction approach, PPP offers low threshold, culturally sensitive, non-judgmental services to address the health and social service needs of drug and hormone users and sex workers in Philadelphia. PPP promotes harm reduction through sterile syringe exchange, mobile medical care, referrals to social services, and prevention case management services.

Medical Case Manager Position Overview:

The Medical Case Manager (MCM) will be working with clients who are enrolled for MCM services through the Division of HIV Health (DHH) and assigned to Prevention Point Philadelphia. The MCM team provides a full range of case management services to participants who are living with HIV and other chronic conditions to support engagement in medical care and medicine adherence.

Position Duties & Responsibilities:

Provide medical case management for individuals and families infected/affected by HIV by assessing clients’ needs and developing, implementing and monitoring service care plans.
Manage a caseload of 25-30, with varying levels of need.
Link clients to appropriate medical, mental health, addiction and support services
Coordinate and monitor services; assist clients with advocating for entitlements and public benefits.
Conduct Level of Care Assessments for clients seeking MOUD treatment referrals.
Provide on-site case management in clinic or hospitals as needed and accompany clients to medical visits.
Conduct home visits as necessary.
Collaborate with other service providers and agencies.
Provide information, education and referral to clients and members of their support system as required.
Adhere to PPP's harm reduction approach (meeting people where they are and withholding judgment about choices they make; acknowledging people’s decisions about their own health).
Assist in keeping accurate and comprehensive charts, including information on participant interviews, counseling sessions, referrals, and submit paperwork in a timely fashion. Perform data entry using internal databases, electronic health record, and CareWare. Provide services in accordance with standards set by PPP and our funders, including the Division of HIV Health.
Help with red carpet navigation and case management in Sana Clinic, a comprehensive HIV primary care clinic at Prevention Point Philadelphia.
Actively participate in team and staff meetings and supervisions.
Complete written documentation of client interactions in DAP format within two business days
Enter and update units of service and demographic information in Careware in a timely fashion in order to fulfill program data requirements.
Perform other duties as assigned by supervisor.

Qualifications

Bachelor’s degree in public health, social work, psychology, or a related field preferred.
A minimum of 2 years of experience working in the field of case management, medicated assisted treatment, overdose prevention, or other related public health.
Experience with or interest in working with individuals experiencing homelessness impacted by substance use
Demonstrated commitment to advocacy and social justice, particularly as related to harm reduction, HIV/AIDS, poverty, and homelessness.
Demonstrated understanding of the principles of epidemiology, surveillance, public health education and social determinants of health disparities.
Spanish speaking preferred.

Skills

Cultural competence required; excellent interpersonal skills.
ASAM and Trauma Informed Care training a plus (will be arranged by PPP if needed).
Ability to use a harm reduction approach with people who use drugs and sex workers.
Ability to utilize electronic health database to enter notes and run reports to collect data.
Dynamic, creative, flexible, able to learn and adapt in a changing environment.
Excellent written communication and organizational skills.
