General Summary
Provides group and individual Diabetes Education. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan of care with each program participant.

Duties and Responsibilities
Essential Functions:

Provides group education and individual education in Diabetes Education.

Provides nutritional counseling to clients referred by a provider, as assigned.

Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.

Triages, manages, and monitors blood glucose levels, medications, continuous glucose monitor patterns, and insulin pumps with the program participant and health care team.

Collects program participant assessment data according to the Diabetes Education Program Key Performance Indicators. Participates in evaluating outcome measures and continuous program improvements for the Diabetic Population. Participates in auditing the program to ensure meeting the program objectives and requirements.

Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.

Utilizes a team approach to provide services and collaborates and communicates with team members when needed.

Participates in peer review process to evaluate performance of other professional staff.

Supports clinical team members by providing essential education and collaboration on the care of the program participant.

Works within their respective scope and practice within the Diabetes Education Program as defined by organizational competencies, policies, procedures, and standard of work.

Supports community events as identified.

Ability to develop a collaborative, therapeutic alliance with individuals in managing persons with diabetes. Applies baseline knowledge of diabetes treatment and management.

Qualifications
Minimum Education:
-
Bachelors Degree Degree in Nursing, Clinical Nutrition or related field. Required
Work Experience:
-
2 years Outpatient, inpatient or other relevant experience. Required
Licenses:
-
Licensed Registered Nurse Upon Hire Required or
-
Registered Nurse Multi State License Upon Hire Required or
-
Licensed Dietitian-Nutritionist within 180 days Required and
-
Registered Dietitian / Registered Dietitian Nurtitionist within 90 days Required and
-
Heartsaver CPR/AED within 90 days Required and
-
Certified Diabetes Care and Education Specialist within 3 years Required
