Duties and Responsibilities
- Conduct comprehensive wound assessments, including wound measurements, documentation, and evaluation of healing progress utilizing EMR/EHR systems.
- Administer wound care treatments, including dressing changes, debridement assistance, and advanced wound care therapies in accordance with evidence-based practices and regulatory standards.
- Manage medication administration related to wound care, including topical treatments and injections, while ensuring accurate documentation and compliance with HIPAA regulations.
- Collaborate with physicians, nurse practitioners, case managers, rehabilitation staff, and other healthcare professionals to develop individualized care plans.
- Monitor vital signs and perform focused patient assessments to identify changes in condition, signs of infection, or increased pain levels.
- Educate patients, families, and caregivers on wound management, infection prevention, mobility, nutrition, and other aspects of care that support healing.
- Maintain accurate and timely medical records in compliance with facility policies, state and federal regulations, and documentation standards.
- Participate in quality improvement initiatives aimed at enhancing wound healing outcomes and reducing preventable skin injuries.
- Support the facility's skin and wound care program through ongoing assessment, prevention, and treatment strategies.
Qualifications
- Current and unrestricted LPN license in the state of practice.
- Current CPR/BLS certification (or ability to obtain upon hire).
- Previous experience in wound care, long-term care, skilled nursing, hospital, home health, or related clinical settings preferred.
- Experience in long-term care is strongly preferred.
- Proficiency with EMR/EHR systems for documentation and patient care management.
- Knowledge of wound care principles, infection control practices, aseptic technique, and skin integrity management.
- Experience caring for geriatric patients and individuals with chronic or complex medical conditions.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and interpersonal skills with the ability to educate patients, families, and interdisciplinary team members.
- Ability to prioritize tasks, work independently, and effectively manage multiple patients in a fast-paced environment.
