Full Job Description

Position Summary:

The Coordinator will use their unique position as a well-trained community member to coordinate substance use services within the SAMHSA CSAT program. The Coordinator will also assist in the navigation of clients to substance use services with a heighten focus on programmatic and census growth. This includes substance use community development, coordinating substance use assessments, facilitating PATH case management assignments to Patient Navigators, providing evidence-based counseling strategies, group facilitation and overall coordination of clients into the program.

Reporting to the Manager of Substance Use Services and Overdose Prevention, the Coordinator will have the following primary responsibilities:

Facilitate the coordination of case assignments to Patient Navigators
Collaborate with program team members on addressing the social needs of assigned clients
Perform biopsychosocial assessments and support with enrollment into the SAMHSA CSAT Program.
Adhere to SAMHSA reporting and outcome tool guidelines
Provide substance use counseling in 1 on 1 and group modalities.
Promote substance use programming internally and with external stakeholders (substance use programs, medication-assisted treatment programs, mental health clinic, CBOs, and other organizations that may encounter individuals who use substances).
Coordinates day to day activities under the supervision of the Manager of Substance Use Services.
Monitors program outcomes, grant program service requirements, and addresses programmatic under performance with treatment team.
Coordinates internal referrals to include cross referring to HIV Primary Care, Case Management, Mental Health Services, and other internal programs.
Supervises Patient Navigators and consults Manager on performance challenges.
Monitors treatment plans and client level outcomes.
Work with vulnerable populations to engage in health literacy, HIV/HCV/STI testing, and health promotion.
Perform required entry of client-level data and documentation of services, including HIV/STD results, linkage to appropriate medical care, referrals to behavioral and social services, biopsychosocial assessments, treatment plans, ASAM, progress notes, transfer/discharge summaries, and other programmatic documentation.
Meet regularly with an interdisciplinary team to discuss census, program service delivery, and team challenges.
Perform other duties as assigned.

Preferred Knowledge, Skills, and Abilities:

Familiar with substance use programs and services within the Washington, DC and suburbanareas.
Ability to grow program census and client engagement
Excellent understanding of substance use disorders, counseling strategies in both 1 on 1 and group formats.
Effective communication with external referring agencies and internal referring team members.
Ability to interpret grant work plans and implement program policies and procedures.
Willingness to engage the team in complex problem solving and programmatic accountability discussions,
Sensitive to others' needs and feelings, especially vulnerable adults; honest and ethical reliable, responsible, and dependable; and willing to take on responsibilities and challenges.
Plan and organize work in an effective and timely manner and, in a team environment,maintain a flexible schedule in accordance with program and organizational needs.

Required Education, Experience, and Qualifications:

● Preferred: ADT (MD), CAC I or II, (DC) LGSW or LGPC Preferred

● Required: License eligible in Maryland and District of Columbia

● Minimum of two years of experience providing substance use services within a community-based setting.

● Bilingual English/Spanish and/or other languages strongly preferred.

Pay: $60,000.00 - $75,000.00 per year
