Full Job Description

Summary:
The Therapeutic Mentoring strives to connect at-risk youth with adult therapeutic mentors (Mental Health Rehabilitation Specialist) who serves as a positive role model, provides support and assistance, and engages the child in enriching community-based experiences. The goal of our program is to assist disadvantaged youth develop coping skills in order to navigate some of the most challenging life circumstances. Our Therapeutic Mentor works one-on-one with the youth, its family and individual therapist to implement treatment goals under the supervision of a behavioral Health Specialist.

Job Responsibilities:

Assists the youth in communicating his/her needs to the treatment team when ageappropriate.
Assists with developing, monitoring, and implementing an appropriate treatment plan with measurable outcomes;
Develops and maintains policies and procedures relating to all components of Therapeutic Mentoring Services;
Offers services in the youth’s home and community;
Delivers services in accordance with an existing outpatient or In-home therapy treatment plan that is jointly developed by the outpatient, or In-home therapy provider with input from the parent/guardian/caregiver, and youth whenever possible;
Supports, coaches and trains the youth in order to support linkages to community resources and services that will sustain the youth’s optimal functioning in the community;
Supports, coaches and trains the youth in connecting with existing treatment providers and in linking with new or additional resources.

Participates with State agencies with regards to service/care planning and coordination and planning for transition to adulthood;
Contributes the voice of the youth in the absence of the youth;
Attends personnel meetings including (wrap-around meetings, staff meetings, and inservice trainings and supervisions);
Collaborates closely and effectively with all other members of the MDT;
Elaborates the documentation of all contacts made with clients and collaterals;
Monitors and documents client progress;
Participates in quality assurance and improvement projects related to the program;
Participates in program planning and expansion services;
Other relevant duties will be completed as assigned.

Job Requirements:

Must be 21 years of age or older;
A bachelor’s degree in a humans services(Social Work, Psychology, Sociology, Criminal Justice, Education, Human Development) field from an accredited university and one (1) year experience working with children/adolescents/transition age youth;
An associate’s degree in a human services (Social Work, Psychology, Sociology, Criminal Justice, Education, Human Development) field from an accredited school and one (1) year of experience working with the target population; or
with children/adolescents/transition age youth;
Ability to work as an integral member of a multidisciplinary team;
Ability to work with racially diverse clients is a must;
Excellent communication and writing skills. Knowledge of child development models, DSM IV diagnoses, crisis intervention and behavior management techniques.
