Full Job Description

North Atlanta Endocrinology & Diabetes is seeking an experienced Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES) to support our Bethlehem and Lawrenceville locations. In this role, you will empower individuals with chronic diseases, including diabetes, to effectively manage their conditions through tailored group and individual diabetes education, medical nutrition therapy, ongoing support, evidence-based care plans, and personalized nutritional guidance. Working closely with our care team, you will play a vital role in improving health outcomes and quality of life for patients with compassionate and evidence-based care.

Key Responsibilities:

Willingness to work in any office location based on needs or organization.
Participate as member of nutrition care team in the provision of evidence-based MNT.
Assess patient needs, establish measurable goals, and monitor progress through consistent follow-up appointments.
Craft individualized, compassionate meal plans based on member health needs and access.
Maintain list of community-based resources in accordance with ADA standards of care.
Collaborate with the broader care team to reinforce clinical goals and SMART goal progression.
Familiarity with continuous glucose monitors (CGMs), insulin pumps, insulin patches, injectable therapeutics and behavior change theories.
Motivated learner, willing to acquire proficiency with the necessary technology (device software applications, EMR, phone systems); experience in Motivational Interviewing to support sustainable behavior change.
Critical thinker with ability to work independently, possess a positive attitude, and the ability to excel in a fast-paced atmosphere while collaborating with diverse groups at all levels of the organization.
Ensure accurate documentation of patient interactions and care plans, adhering to HIPAA regulations and organizational policies.
Collaborate with physicians, dietitians, and other healthcare professionals to create and implement personalized care plans that address each patient’s unique needs.
Stay informed about the latest advancements in diabetes care and treatment, sharing knowledge with patients, colleagues, and staff as appropriate.
Assess patient health regimens and interpret blood glucose and CGM readings as they relate to nutrition.
Deliver individualized and group education sessions on diabetes self-management, covering topics such as nutrition, medication adherence, blood glucose monitoring, and lifestyle modifications.
Support organization in meeting ADA DSME Education Recognition Program standards:
Standard 2: Engage in community outreach initiatives to promote diabetes awareness, prevention, and education.

· Standard 3: Maintain RD, LD, and CDCES with credentialing organizations.

· Standard 4: Support the development, maintenance and delivery of DSME services in individualized and group diabetes education settings, incorporating cultural preferences and learning styles.

· Standard 5: Deliver person centered DSME, addressing barriers to effective self-care and help patients develop problem-solving and coping strategies.

· Standard 6: Monitor patient progress and adjust education plans as needed. Maintain accurate documentation and contribute to program evaluation and development. Participate in quality improvement initiatives and community outreach events.

Pay: $30.00 - $35.00 per hour
