Full Job Description

Qualifications

1. High School Diploma or GED Equivalent

2. 21 years of age

3. Must successfully complete the Medication Administration course within the past 2 years.

4. Must successfully complete the Performance-based competency test within the past 2 years.

5. Must successfully complete the Diabetes Patient Education Program within the past 12 months.

6. Evidence of good physical and mental health. Must be free of medical conditions that limit the staff member’s ability to provide the necessary direct care services with reasonable skill and safety. This includes, but is not limited to, drug and alcohol addiction as specified in § 2600.54(a)

7. Show an interest and caring attitude toward residents while exhibiting a warm and welcoming personality.

8. Must respect and show concern for all residents regardless of age, race, religion, gender, nationality or mental status.

9. Willingness to accept and learn from supervision. These include the ability to learn and execute the procedures as instructed by the staff member whom is training.

10. Willingness to abide by Our Home of Hope Rules.

Abilities:

1. Ability to bend, lift and transfer residents.

2. Ability to interact with the residents, families and co-workers.

3. Ability to work independently.

4. Ability to deal with emergencies calmly and efficiently.

5. Ability to effectively utilize time.

6. Ability to read, write and speak English.

a. Must be able to record messages and keep records appropriate to job assignments

7. Ability to make significant observations of functional problems related to the resident or the facility.

8. Ability to be flexible with schedule.

Responsibilities:

1. Administration of oral; topical; eye, nose and ear drop prescription medications and epinephrine injections for insect bites or other allergies.

2. Administration of insulin injections.

3. Responsible for the ordering and maintaining of Prescription Medications.

4. Responsible for making any changes requested by a resident’s physician.

5. Responsible for making sure all Medications are labeled properly and with original pharmacy labels.

6. Responsible for making sure all Medication Administration Records (MARs) are current and up to date.

7. Responsible for the transferring of MARs for each new month.

8. Responsible for assuring the Medication Cart is locked at all times when not in use.

9. Responsible for assuring that all Medication stored in a refrigerator is kept in an area or container that is locked.

10. Responsible for assuring that all discontinued medications and medications from residents whom are no longer living in the facility are returned or disposed of in safe manner according to the Department of Environmental Protection and Federal and State regulations.

11. Responsible for making sure Medications are stored in an organized manner under proper conditions of sanitations, temperature, moisture and light in accordance with the manufacturer’s instructions.

12. Must contact resident’s physician or specialist when a refusal of medication has occurred.

13. Responsible for the reporting of all medication errors.

a. Must report immediately to the resident, resident’s designated person and the prescriber of the medication.

b. Documentation of the error and the prescriber’s response shall be completed with a medication error form.

c. The administrator should be notified immediately so that an incident report can be completed.

14. Must Evaluate, Observe and preform Safety Checks medication supplies of resident’s whom self-administer medications.

a. Evaluations and Observations should be completed every three months unless significant changes in the resident’s ability to self-administer are detected.

b. Reminders may be given daily to assure medications are being taking properly.

c. Safety Checks should be completed Weekly or Bi-Weekly to assure that all medication is be kept in the provided lock box.

15. Responsible for keeping all medication logs current.

16. Responsible for keeping medication cart clean and free from debris.

17. Assist residents with shaving, oral hygiene, nail care and grooming.

18. Assist residents with dressing/undressing as needed.

19. Assist with serving of meals.

20. Cue residents to follow a daily schedule of appointments.

21. Help maintain cleanliness and neatness of resident rooms and common areas.

22. Assist residents in getting to meals and activities.

23. Assist or cue residents in dressing appropriately for the weather.

24. Report any changes in medical statuses of a resident to administrator.

25. Report documentation of daily tasks on appropriate forms.

26. Complete change of shift reports each shift and counting of narcotic medication between shifts.

27. Assist with any treatments that need to be completed on a resident.

28. Attend all mandatory staff meetings or resident council meetings upon the request of administrator.

29. Assist with activities as requested or required by supervisory staff.

30. Follow other instructions and perform any related job duties that may be required for the position, etc.
