Full Job Description

Legacy Healing Center Ohio is a premier Dual Diagnosis treatment facility committed to providing compassionate, high-quality care to individuals on their mental health/recovery journey. We are currently seeking a dedicated and organized Case Manager to support our Cincinnati,OH. location.

This is an in-office position requiring you to work the following schedule:

Monday-Friday 8AM-4:30pm, on call every third Saturday for a couple of hours for any weekend intakes

Position Overview

The Case Manager plays a vital role in coordinating care, supporting client needs, and ensuring continuity throughout treatment. This position works closely with clinical teams, external providers, and clients to facilitate successful treatment outcomes and smooth transitions of care.

Key Responsibilities

Coordinate and manage client care plans from admission through discharge
Assist clients with resources related to housing, employment, legal matters, and aftercare planning
Maintain accurate and timely documentation in compliance with regulatory and company standards
Communicate effectively with clinical staff, admissions, utilization review, and external partners
Identify patient care issues and support clinicians with the implementation of new clinical pathways to improve quality of care.
Develop a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice.
Facilitate referrals and follow-up appointments as needed
Advocate for clients while maintaining professional boundaries and ethical standards

Qualifications

Experience working in a for-profit facility.
Experience with extending FMLA/short term disability, filing for COBRA paperwork, etc.
2 years prior case management experience in substance abuse or behavioral health
Previous experience with psychological aspects of care
KIPU experience
Reliable transportation
Strong organizational, communication, and time-management skills
Ability to work independently in a fast-paced, compliance-driven environment
Bachelors Degree

Schedule & Compensation

Full-time, in-office position
Schedule: M-F, on call every third Saturday
Salary: $50,000-$55,000 annually
