We are looking to immediately contract for a: Hospice RN - Admit/Flex Nurse - 3pm to 7pm, M-F
Pay Rate: Will be a base plus a per visit rate. Negotiable based on experience.
Job Summary
The Hospice RN - Admit/Flex Nurse is responsible for assisting with hospice admits that occur M-F around the hours of 3pm-7pm. This position will also be responsible for assigned PRN visits from the DON. Responsibilities include initial and on-going assessment of patients, developing and revising the plan of care, timely documentation, medication management, communication of assessments, planning and interventions and coordination of care among team members.
Essential Responsibilities
- Performs initial and ongoing nursing evaluations of patients/family/caregiver and assists in the development/implementation of the plan of care. Nurses may be responsible for one or more of the following areas:
- Admissions
- Initial hospice assessments
- Medication management
- Routine or focused nursing visits
- Telephonic triage
- Other duties assigned by the Hospice Leadership
- Performs duties in compliance with Medicare Condition of Participation.
- Responsible to coordinate and monitor all care being provided and keeps the physician and other team members from agency and community resources apprised of changes in patient’s condition so that adjustments can be made to the plan or care as needed, to improve quality of life.
- Uses clinical assessment skills and best practices to identify and provide effective care for patients, families and caregivers.
- Updates and implements plan of care changes when necessary during on-call visits and communicates those changes with the care team as well as the patient/family/caregiver unit.
- May assist patient/family/caregiver in determining of goals of care including completion of Advance Directives and POLST forms as needed.
- Provides dietary counseling as needed.
- Address advocacy needs for patients/families/and organization.
- Facilitates formal and informal meetings with patients/caregivers/family member as needed.
- Collaborates with the IDT during crisis management support and consultations with patient/family/caregiver or IDT team.
- Provides information regarding the patient/family/caregivers psychosocial, spiritual, physical and financial needs or difficulties to the IDT team or providers involved in the patient’s care.
- Serves as liaison with hospitals, care facilities and appropriate agencies to promote interagency cooperation, developing and maintaining communication and facilitating referrals to meet patient needs throughout the continuum of care.
- Participates in continuing education of self and assists with orientation of other team members.
- Collaborates with Serenity Hospice Providers program staff to enhance continuity of care as needed.
- Responsible for documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards per policies and procedures.

Experience and/or Education
Required
- Current unencumbered license as a registered nurse in the State of Utah
- Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
- CPR certification at hire or within 6 months in position
Preferred
- Bachelor of Nursing degree or higher
- Certified in Hospice and Palliative Care (CHPN)
- Experience in hospice triage, case management, admissions, night/weekend roles
- 2 years’ medical/surgical experience in an acute care setting and/or a combination of medical/surgical, home health, hospice experience or long-term care experience
Knowledge, Skills and Abilities Required

- Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations
Skills and Abilities
- Ability to perform nursing related tasks in the area of specialty (e.g., case management, on-call triage management, visits, and/or phone calls in a home care setting
- Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
- Must have general computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internet

- Ability to operate a motorized vehicle in all kinds of weather conditions
Working Conditions
Work Environment(s): Indoor/Office Community Facilities/Security Outdoor Exposure
Member/Patient Facing: No Telephonic In Person
Hazards: May include, but not limited to, physical, ergonomic, and biological hazards.
Equipment: General office equipment and/or mobile technology
Travel: Requires travel outside of the workplace at least weekly; the employee’s personal vehicle may be used. Driving infractions will be monitored in accordance with organizational policy.
Schedule: This position may occasionally require more than 8 hours per day
