Full Job Description

Position Overview: The Case Manager will provide case management and advocacy to clients assigned to them. The primary focus of this position is to support clients in their efforts to gain housing stability, financial stability, and life skills development to prevent and end homelessness.

Essential Job Functions:

Complete assessment of clients to determine strengths and needs
Recommend goals and develop an Individual Service Plan for each client
Ensure that clients are connected to the correct services in the community to successfully meet each goal
Maintain frequent contact with clients to monitor progress with goals and to ensure compliance with Three Oaks Center’s expectations
Assist clients with finding and maintaining affordable housing to prevent and end homelessness
Work with clients to create reasonable monthly budgets, ensure clients can and do pay monthly expenses and spend within budget constraints
Maintain accurate case files
Utilize database to track and report client data and outcomes
Communicate, collaborate with, and support all programs and staff at Three Oaks Center
Build working relationships with partnering community agencies

Job Requirements and Skills:

Minimum high school diploma
College Degree or 2 years’ experience preferred
Must have reliable transportation
Must be an excellent verbal and written communicator
Must be a proactive critical thinker and problem-solver

Job Type: Full-time

Pay: $42,000.00 per year
