Full Job Description

Job Overview
We are seeking a dedicated and compassionate Licensed Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to patients across various settings, including hospitals, outpatient clinics, and community-based programs. Your expertise will support individuals and families navigating complex health challenges, mental health issues, and developmental disabilities. This position offers an exciting opportunity to make a meaningful difference in patients’ lives through clinical counseling, case management, and crisis intervention. The ideal candidate will be energetic, detail-oriented, and committed to delivering exceptional patient-centered care.

Duties

Conduct thorough patient assessments to evaluate social, emotional, and environmental factors affecting health and well-being
Provide individual and group psychotherapy sessions utilizing behavioral therapy, cognitive behavioral therapy (CBT), motivational interviewing, and other evidence-based approaches
Assist with intake procedures, documentation, and medical records management in compliance with HIPAA regulations
Develop personalized care plans that incorporate social work best practices for patients with mental health conditions, developmental disabilities, dementia, or post-acute care needs
Collaborate with multidisciplinary teams including medical staff, therapists, and community resources to coordinate comprehensive patient care
Facilitate crisis intervention and manage urgent situations with professionalism and sensitivity
Support patients through discharge planning, home & community care arrangements, hospice services, or assisted living placements

Requirements

Valid licensure as a Licensed Social Worker (LSW or LCSW) in the relevant state or jurisdiction
Proven experience in hospital settings or outpatient clinics; experience with inpatient or nursing home environments is highly desirable
Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and EMR systems for accurate documentation and data collection
Expertise in child & family counseling, behavioral health interventions, addiction counseling, Alzheimer's care, PTSD care, dementia care, autism spectrum disorder support, and geriatrics is preferred
Familiarity with crisis management techniques and working with individuals with disabilities or developmental delays
Excellent communication skills for conducting diagnostic evaluations, case management activities, and working with diverse populations including children and seniors
Ability to handle sensitive situations with empathy while maintaining compliance with HIPAA standards
