Job Overview
We are seeking a dynamic and compassionate Adult Nurse Practitioner to join our multidisciplinary healthcare team. In this vital role, you will deliver high-quality, patient-centered care across diverse clinical settings, including outpatient clinics and skilled nursing facilities. Your expertise will help improve health outcomes by providing comprehensive assessments, diagnosing conditions, developing treatment plans, and managing ongoing patient care. This position offers an exciting opportunity to make a meaningful difference in the lives of adults with varied healthcare needs while working within a supportive and innovative environment.

Duties

Conduct thorough physical examinations and patient assessments to determine health status and identify medical issues.
Develop and implement individualized treatment plans.
Manage acute and chronic conditions such as pain management, infectious diseases, geriatrics, behavioral health, and post-acute care needs.
Utilize electronic health record (EHR) system eClinicalWorks to document patient encounters accurately and efficiently.
Perform diagnostic evaluations using laboratory testing, sonography, medical imaging, and specimen collection/processing techniques.
Provide patient education on health coaching topics such as nutrition, disease prevention, medication adherence, and lifestyle modifications.
Collaborate with specialists in areas like gastroenterology, cardiology, ophthalmology, and occupational health to coordinate comprehensive care.
Ensure compliance with HIPAA regulations and maintain meticulous medical documentation for legal and billing purposes using CPT coding and ICD coding standards.

Skills

Proven experience working in outpatient and skilled nursing settings.
Strong knowledge of internal medicine and long term care.
Skilled in managing complex cases involving acute pain management, infectious disease care, hospice & palliative medicine or geriatrics.
Proficiency with EMR/EHR systems like eClinical works is a plus.
Familiarity with diagnostic tools like sonography or medical imaging; experience with laboratory testing processes is a plus.
Excellent patient assessment skills complemented by physiology knowledge and clinical research experience when applicable.
Capable of working across multiple settings including outpatient clinic and skilled nursing settings.
Strong communication skills for effective collaboration with multidisciplinary teams including behavioral health specialists or memory care providers.
