Position Summary
The Social Services Coordinator supports the social, emotional, and psychosocial needs of residents and their families throughout their stay. This position serves as an important connection between residents, families, team members, and outside resources to promote resident well-being, dignity, choice, and quality of life.
The Social Services Coordinator works closely with nursing, therapy, activities, admissions, and other members of the interdisciplinary team to identify resident needs, address concerns, assist with transitions, and ensure residents and families feel supported and informed.
Hours: Full Time Day Hours, 64 hours every 2 weeks, no Holidays, no Weekends.
Essential Duties and Responsibilities
Develop positive, supportive relationships with residents and their families.
Assist residents and families with adjustment to admission, changes in health or function, room changes, and other transitions.
Identify and address residents' social, emotional, and psychosocial needs.
Participate as an active member of the interdisciplinary care team.
Participate in resident care conferences and assist residents and families in understanding and participating in the care-planning process.
Communicate resident and family concerns to appropriate team members and assist with timely resolution.
Advocate for residents' rights, preferences, dignity, independence, and quality of life.
Assist residents and families with advance directives and other appropriate healthcare planning resources.
Assist with discharge planning and coordination of services necessary for a safe transition to home or another care setting.
Coordinate referrals and communication with community agencies, healthcare providers, and other outside resources as appropriate.
Provide support and resources to residents and families experiencing significant health changes, loss, grief, or other difficult transitions.
Assist residents in maintaining important family and community relationships.
Participate in behavioral and psychosocial care planning when appropriate.
Maintain accurate and timely documentation in the resident's medical record according to facility policy and applicable requirements.
Assist with resident and family grievances and ensure concerns are communicated and followed through appropriately.
Support resident rights and help ensure residents are treated with dignity, respect, and compassion.
Maintain confidentiality and follow HIPAA and facility privacy requirements.
Participate in quality improvement initiatives related to resident experience and psychosocial well-being.
Collaborate with leadership and other departments to promote a positive resident and family experience.
Perform other duties related to resident social services and support as assigned.
Education and Experience
A degree in social work is not required for this position.
Candidates with education or experience in human services, healthcare, psychology, sociology, gerontology, social services, nursing, or a related field are encouraged to apply.
Experience working with older adults, individuals with healthcare needs, residents, patients, or families is preferred. Equivalent combinations of education, training, and relevant experience may be considered based on the needs and regulatory requirements of the position.

Preferred Qualifications
Previous experience in long-term care, assisted living, healthcare, human services, case management, or a related setting
Experience working with older adults and their families
Familiarity with community resources and healthcare services
Experience with care planning, discharge planning, or resident advocacy
Position Expectations
The Social Services Coordinator is expected to demonstrate compassion, professionalism, confidentiality, dependability, and strong communication while supporting residents and families.
This position should consistently promote a resident-centered approach that recognizes each resident as an individual and supports their physical, emotional, social, psychological, and spiritual well-being.
Pay: $25.00 - $38.00 per hour
