Engaging Arkansas Communities (EAC) is seeking a Medical Case Management Coordinator to join our growing team. This position plays a vital role in improving health outcomes by coordinating medical case management services, linking clients to care, supporting retention in care, and helping individuals navigate the healthcare system.

Position Summary
The Medical Case Management Coordinator is responsible for coordinating comprehensive prevention and medical case management activities that improve access to healthcare and supportive services for individuals across Arkansas. Working collaboratively with Arkansas Department of Health, healthcare providers, community organizations, and EAC staff, this position helps connect clients to care, reduce barriers to services, and improve health outcomes through education, outreach, and care coordination.
What You'll Do
- Conduct HIV, STI, TB, and Hepatitis C prevention and education activities.
- Coordinate linkage to care and retention in care services.
- Perform disease investigations and partner services.
- Conduct community outreach, education, and testing events.
- Build partnerships with healthcare providers and community organizations.
- Connect clients to medical care, housing, behavioral health, transportation, and supportive services.
- Maintain accurate documentation and program data.
- Assist with grant objectives, quality improvement activities, and program reporting.
- Travel throughout Arkansas to serve communities where services are needed.
Preferred Qualifications
We're looking for candidates who have:
- Bachelor's degree in Public Health, Social Work, Human Services, Healthcare Administration, Nursing, Business Management, or a related field (or an equivalent combination of education and experience).
- Experience in healthcare, case management, public health, patient navigation, community outreach, or disease intervention preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently and manage multiple priorities.
- Valid driver's license, reliable transportation, and willingness to travel statewide.
