GENERAL DESCRIPTION
Central City Health is committed to enabling every patient in our community to thrive by building a collaborative multidisciplinary healthcare team. The MD is a key member of the clinical team responsible for delivering high-quality, comprehensive adult primary care in an outpatient setting. The MD will assess, diagnose, treat, and manage acute and chronic illnesses, emphasizing preventive health strategies and patient education. The ideal candidate is committed to health equity and passionate about improving access to care in nonprofit and community-based settings.
RESPONSIBILITIES
1. Provide direct, patient-centered care in an outpatient clinical setting.
2. Perform comprehensive and problem-focused medical evaluations, histories, physical examinations, and formulate treatment plans.
3. Diagnose and manage a wide range of chronic and acute medical conditions not limited to hypertension, diabetes, COPD, cardiovascular disease, obesity, and infectious diseases.
4. Provide preventive care and health screenings in accordance with (e.g., USPSTF).
5. Order, interpret, and follow up on diagnostic tests (e.g., labs, imaging, ECGs), and make referrals as clinically indicated.
6. Provide clinical leadership/consultation and guidance to members of the interdisciplinary medical team, including advanced practice providers and nursing staff, etc.
7. Prescribe medications appropriately, considering formulary limitations and cost-effectiveness.
8. Collaborate with behavioral health, care coordination, and specialty providers to support holistic, team-based care.
9. Provide culturally competent care sensitive to the unique needs of the metro Detroit community.
10. Utilize the electronic health record (EHR) system for accurate and timely documentation, adhering to organizational and regulatory standards.
11. Participate in integrated care initiatives, collaborating with behavioral health, dental, pharmacy, care management, and other internal departments.
12. Follow all Central City Health protocols, clinical policies, and compliance with HRSA, FQHC, and other regulatory agencies’ standards.
13. Educate patients and their families on health conditions, treatment options, medications, and preventive strategies.
14. Maintain active licensure and complete required continuing medical education (CME) timely.
15. Participate in call coverage and extended hours on a rotating basis, if needed.
16. Establish and maintain ongoing physician-patient relationships to promote continuity of care across the healthcare continuum.
17. Provide comprehensive longitudinal primary care to adult patients, consistent with the physician’s training, clinical privileges, and scope of practice.
18. Participate in population health, quality improvement, and clinical outcomes initiatives designed to improve patient health and reduce health disparities.
19. Perform other duties as assigned.
EDUCATION AND EXPERIENCE
- Doctorate degree in Medicine (MD or DO) required.
- Unrestricted Michigan medical doctor/doctor of license and controlled substance license required.
- Valid DEA certification required
- Board certification or board eligibility in Family Medicine, Internal Medicine
- Michigan Driver’s license required.
- BLS certification required (ACLS preferred).
- Must meet all Continuing Education requirements.
- Experience interacting with mental illness patient population.
- Prior experience working in a community mental health or FQHC setting with a patient centered integrated health centered approach preferred.
- Clinical risk management experience preferred.
- Work cooperatively with all involved in the delivery of service to patients
- Interpret laboratory results and imaging studies to support clinical decision-making.
- Knowledge of evidence-based medicine and clinical guidelines.
- Experience within an integrated care system is strongly preferred.
- Experience in managing multiple tasks simultaneously and meeting critical deadlines.
- Maintain confidentiality relating to treatment in accordance with HIPAA guidelines.
- Strong interpersonal skills, high emotional intelligence; hands-on when necessary,
- Proficient in the use of EHR systems for patient documentation and management
- Thorough knowledge and understanding of current trends in health care, including physician hospital relations, physician practice models, access issues, quality improvement processes, clinical information systems, and the implications of changing health care environment for medical credentialing are essential.
Qualified Mental Health Professional (QMHP) - Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant OR a human services professional with at least a bachelor’s degree in a human services field.
Qualified Behavioral Health Professional (QBHP) – starting January 1, 2020, a QBHP must be certified within two years of successfully completing their ABA graduate coursework or by 9/30/2025 whichever is the shorter time-period.
Must be a physician or licensed practitioner (e.g. Advanced Practice RN, Psychologist, Clinical Social Worker, Physician Assistant, etc.) with specialized training and one year of experience in the examination, evaluation, and treatment of children with ASD.
Health Care Professional - Health Care Professional – A physician, registered nurse, physician assistant, nurse practitioner, clinical nurse specialist, or dietician. Services provided must be relevant to the health care professional’s scope of practice.
PHYSICAL DEMANDS/WORKING CONDITIONS:
- Time spent in this position may be split between being in a clinic, attending meetings, events, conferences/seminars and programs at other locations.
- Walking, sitting, and/or standing for prolong periods of time may be required.
- Interacts with a patient population exhibiting severe and persistent mental illness.
- Normal office and medical environments with exposure to noise, dust, or extreme temperatures.
- Must be available to work all hours scheduled. These include but are not limited to early mornings, evenings and weekend hours as determined by the organization and Senior Leadership.
- Work hours may shift depending on project or program needs.
- May have close contact with clients and/or their living environments which could include potential exposure to contagious diseases, unpleasant odors and/or to bug/pest infestations.
- Must possess and maintain an acceptable background check clearance, certifications, licensures and/or training as required by Central City Health, federal/state/county agencies, and other entities
