Full Job Description

Title: Program Supervisor – PRP/Targeted Case Management (LCPC, LCSW-C, LMSW, LGPC, PMHNP, RN)

FLSA Status: Non-Exempt

Department: Support Services

General Summary

The Program Supervisor, PRP/Targeted Case Management, plays a pivotal role in ensuring the effective

delivery of PRP/Targeted Case Management in compliance with COMAR regulations. This position provides oversight and leadership to PRP/Targeted Case Management teams, ensuring alignment with best practices and regulatory standards. The supervisor is responsible for maintaining high-quality service delivery, supporting staff development, and facilitating collaboration with other disciplines within the clinical setting.

The Program Supervisor is a key member of the care team, overseeing a comprehensive range of behavioral health care service coordination for children and adolescent populations. This individual ensures that care coordination and prevention services are delivered effectively from intake through discharge, fostering a client-centered approach to treatment and intervention. The Program Supervisor works closely with interdisciplinary teams, delegates appropriately to staff, and directs service provision to meet the needs of clients.

The Program Supervisor is expected to exercise sound judgment in service delivery and personnel management. They promote teamwork among clinical and administrative personnel across multiple service areas, ensuring seamless collaboration. This role requires staying current with best practices and regulatory guidelines through ongoing education relevant to behavioral health and care coordination. Additionally, the Program Supervisor must demonstrate the ability to effectively lead teams, mentor new staff, and provide oversight to ensure consistent implementation of program standards.

This position requires the ability to demonstrate, document, and educate others on key service delivery components, including core behavioral health measures, crisis intervention, service utilization, client engagement strategies, and regulatory compliance. The Program Supervisor must effectively integrate technical competencies with a biopsychosocial approach to care. They must also exhibit strong critical thinking skills in resolving service challenges, managing caseload distribution, and ensuring efficient operations within the clinical setting.

The Program Supervisor upholds established standards for service quality, compliance, and client-centered care while actively engaging in performance improvement initiatives. This individual identifies operational concerns that require administrative intervention and collaborates with leadership to implement solutions. They also participate in interdepartmental committees, workgroups, or planning teams to support the mission and goals of Hope Health Systems, Inc. Through strong leadership and service delivery oversight, the Program Supervisor fosters an environment of excellence in care coordination and prevention services.

Principal Responsibilities and Duties:

The following statements are intended to describe the general nature and level of work being performed by people

assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by

personnel so classified.

1. Oversee the operations of the Care Coordination and Prevention Specialist teams to ensure high-quality

service provision in accordance with COMAR regulations.

2. Ensure documentation and reporting meet COMAR compliance standards for care coordination and

prevention services through daily documentation review practices.

3. Provide direct supervision and support to staff, promoting adherence to policies, protocols, and

performance expectations.

4. Facilitate the onboarding and training of new staff, including oversight of onboarding schedules,

documentation, and training requirements.

5. Develop and implement ongoing training programs to enhance staff competencies and ensure compliance

with regulatory requirements.

6. Collaborate with internal and external stakeholders, including school administrators, community partners,

and healthcare providers, to facilitate integrated service delivery.

7. Monitor and ensure that prevention specialists and care coordinators meet their required caseloads and

deliverables.

8. Conduct regular audits, including documentation reviews, to ensure quality control and adherence to

established standards.

9. Serve as the primary point of contact for addressing program concerns, ensuring swift resolution of issues

affecting care coordination and prevention services.

10. Implement progressive performance management strategies to maintain a minimum 85% staff productivity

rate and support performance improvement initiatives when necessary.

11. Reinforce Hope Health Systems’ mission, vision, values, philosophy, goals objectives and policies.

Staff Development & Performance Monitoring:

1. Conduct regular staff evaluations and performance reviews, providing constructive feedback and targeted

improvement plans.

2. Support staff in professional development and continuous education efforts to enhance service delivery

quality.

3. Ensure compliance with care coordination and prevention service documentation and reporting standards

as per COMAR requirements.

4. Establish clear performance expectations and enforce progressive disciplinary measures when needed.

5. Host monthly staff meetings to provide updates, encourage collaboration, and address operational

concerns.

Patient Safety

1. Ensures patient safety in the performance of job functions and through participation in system or

department level patient safety initiatives.

2. Takes action to correct observed risks to patient safety and appropriate care coordination.

3. Reports adverse events and near misses to appropriate management authority.

4. Implements policies, procedures, and standards consistently in the performance of assigned duties.

Knowledge, Skill and Abilities Required:

1. Knowledge of behavioral health principles, care coordination strategies, and prevention service best

practices acquired through the completion of a Master's degree in Social Work, Public Health, Behavioral Health, or a related field.

2. Minimum of two years of experience in behavioral health, care coordination, or program supervision.

3. Demonstrated knowledge of and compliance with COMAR regulations governing care coordination and prevention services.

4. Maintains professional knowledge and skills through participation in professional meetings, workshops, courses, and in-service training relevant to behavioral health care coordination.

5. Strong organizational, leadership, and communication skills to effectively manage staff, collaborate with interdisciplinary teams, and support client-centered service delivery.

6. Proficiency in Microsoft Teams, data tracking systems, and documentation processes to ensure compliance and efficiency in reporting and performance monitoring.

7. Ability to effectively collaborate with multiple stakeholders, including school systems, healthcare organizations, and community agencies, to enhance service integration and access to care.

8. Must have AHA Basic Life Support/CPR Certification for healthcare providers at time of employment and throughout the tenure of employment.

9. Analytical ability and sound clinical knowledge for effective review and understanding of coordination plans, implementation of care, and appropriate documentation of pertinent information regarding patient’s condition. Individual must be able to identify the need for assistance/supervision and seek out appropriate resources.
