Speech-Language Pathologist (SLP)

PRN | Home Health and Hospice - Location; CNS Layton Office, Serving Davis and Weber County

Position Summary

CNS is seeking a compassionate PRN Speech-Language Pathologist (SLP) to support Home Health and Hospice patients in the Layton area. This role offers meaningful 1:1 care, greater autonomy, and the opportunity to work with a mix of adult and some pediatric patients. The SLP will evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in the home setting, helping patients regain function, independence, and quality of life.

Compensation

Per Visit: $65.00 - $75.00 per visit. Compensation details will be outlined in your offer letter. *Indeed will not allow a per visit rate, so the hourly listed is per visit.
Mileage Reimbursement: Mileage reimbursement is provided at $0.57 per mile, starting after your first patient visit and ending after your last visit of the day. Travel from your home to the first visit and from your last visit back home is not reimbursed.
Classification: PRN, non-exempt hourly employee
Schedule: Flexible schedule, PRN, we’ll work with you to set up a schedule that will work for your and our patients.

Key Responsibilities

Provide excellent care within the scope of your practice and licensure as a licensed Speech-Language Pathologist.
Conduct comprehensive evaluations of speech, language, voice, fluency, cognitive-communication, and swallowing disorders in the home setting.
Develop and implement individualized treatment plans based on patient needs, goals, and physician orders.
Provide dysphagia assessment and intervention, including modified diet and texture recommendations, to promote safe swallowing and reduce aspiration risk.
Deliver therapeutic interventions targeting communication deficits resulting from stroke, neurological conditions, head and neck cancer, TBI, and other diagnoses.
Educate patients, caregivers, and family members on home exercise programs, compensatory strategies, and safe feeding techniques.
Collaborate with physicians, nurses, physical therapists, occupational therapists, and other interdisciplinary team members to ensure coordinated, comprehensive care.
Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
Document care provided and patient observations accurately and in a timely manner in accordance with regulatory requirements and CNS policies.
Maintain patient dignity, privacy, and comfort at all times.
Adhere to all safety, infection control, and HIPAA guidelines.
Participate in quality improvement initiatives and continuing education opportunities as required.

Qualifications

Education & Licensure

Master’s or Doctoral degree in Speech-Language Pathology from an accredited program.
Current Utah State licensure as a Speech-Language Pathologist in good standing.
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) issued by ASHA, or eligibility for certification.
CPR/BLS certification required (or able to obtain within 30 days of hire).

Experience & Knowledge

Experience in home health, hospice, or rehabilitation settings preferred.
Experience with dysphagia evaluation and treatment, including modified barium swallow studies and FEES, preferred.
Familiarity with neurological, oncological, and geriatric patient populations.
Knowledge of Medicare, Medicaid, and home health documentation requirements.
Experience with AAC (augmentative and alternative communication) devices a plus.

Skills & Abilities

Strong communication and interpersonal skills with the ability to build rapport with patients and families.
Ability to work independently with minimal supervision in diverse home settings.
Strong organizational and time management skills with ability to manage a field-based caseload.
Cultural competency and sensitivity to diverse populations.
Proficiency with electronic medical records and documentation systems.
Ability to handle emotionally challenging situations with professionalism and empathy.

Other Requirements

Reliable transportation and willingness to travel to patients in the Layton and surrounding communities.
Valid driver’s license and current auto insurance.
Ability to manage a field-based caseload and flexible PRN schedule.
Comfortable working independently in diverse home environments.
Ability to pass a criminal background check and drug screening.
Current CPR certification (or obtained within 30 days of hire).

Physical Requirements

Ability to lift, transfer, and reposition patients (up to 50 lbs).
Ability to drive throughout the CNS Layton office service area and surrounding communities.
Ability to sit, stand, walk, bend, kneel, and squat for extended periods.
Ability to navigate stairs and various home environments including those with pets, limited accessibility, and varying cleanliness conditions.
Physical stamina to provide hands-on patient care across multiple home visits throughout the day.
Manual dexterity for administering assessments, therapeutic interventions, and documentation.
