Essential Duties and Responsibilities:
The Case Manager's primary objective is to link clients to appropriate community-based care and support services, aiming to prevent their involvement in the criminal justice system and avoid unnecessary institutionalization. By providing access to mental health and substance use treatment, counseling, and social services, the goal is to address clients' underlying issues and promote a healthier and more stable lifestyle. Their primary responsibilities encompass case management, financial management, and life skills services, all aimed at promoting the clients' overall well-being and successful recovery.
- Case Managers must be registered with the DCHS agency or be willing to complete the registration process, as this position requires onboarding to BWell and registration in SmartCare to document client services.
- Connect clients to stable, permanent, and affordable housing as quickly as possible: The objective is to expedite the process of securing stable housing for clients to enhance their stability and reduce homelessness.
- Facilitating referrals to qualified treatment providers and ensuring clients receive appropriate therapies and interventions based on their specific needs.
- Interviews clients with problems such as personal and family adjustments, finances,employment, food, clothing, housing and physical and mental impairment to determine nature and degree of problem.
- Provide individual counseling to residents to address challenges, enhance coping skills, and promote relapse prevention strategies.
- Creates a comprehensive case plan tailored to the client's needs, encompassing both short-term and long-term objectives.
- Establishes a timeline and measurable indicators for each goal within the plan.
- Regularly conducts scheduled weekly sessions to monitor the client's progress towards achieving these goals.
- Engages in effective collaboration with other staff members and programs, fostering a cohesive and coordinated approach to address clients' needs and challenges.
- Transports clients in the agency-provided vehicle for appointments as required.
- Help clients search for available housing options that align with their preferences and needs. Assist in browsing listings, contacting landlords, and submitting applications.
- Works collaboratively with partner organizations and agencies to enhance the range of available resources and support for clients.
- Evaluates and adjusts the case plan as necessary, and issues written warnings with potential consequences if satisfactory progress is not attained.
- Empowers clients to actively participate in their own planning and goal-setting process.
- Refers clients to suitable resources that can aid them in achieving their objectives, such as mental health services, drug and alcohol treatment programs, and mandatory probation/parole attendance groups like Domestic Violence, Anger Management, and Reunification.
- Acts as the central point of contact for each client, facilitating connections to essential resources within the community, including CalFresh, Medi-cal, health insurance benefits, Cash Aid, long-term/permanent housing options, employment opportunities, 12-step meetings, and more.
- Maintain accurate client data in the agency database, ETO, ECM, HMIS and county database EHR daily.
- Conducts UA testing for clients as needed.
- Be prepared to respond to crisis situations promptly and effectively, providing immediate assistance and access to appropriate resources when needed.
- Provides a weekly report to the CSL Program Manager, sharing updates, progress, and important information related to the job responsibilities and activities.
- Responsible for promptly notifying the manager of any work orders that need to be submitted or supplies that need to be ordered.
- Responsible for attending staff meetings, ensuring timely completion of training, and accurately completing timecards.
- Perform related duties as assigned by the supervisor.
- Maintain compliance with all company policies and procedures.
Education and/or Work Experience Requirements:

- High School Diploma or equivalent: A minimum educational requirement to demonstrate basic literacy and communication skills.
- Associate's degree required.
- Minimum one year prior experience in case management, social work, mental health services, substance abuse treatment, or related fields is highly valuable. Experience in working with vulnerable populations, such as homeless individuals, those with mental health challenges, or substance use disorders, is particularly beneficial.
- Excellent communication skills: Ability to effectively communicate with shelter residents, colleagues, and external stakeholders.
- Crisis intervention skills: Capacity to handle crisis situations and de-escalate conflicts.
- Empathy and compassion: Demonstrating genuine care and understanding for individuals experiencing homelessness.
- Ability to work in a team: Collaboration and cooperation with other shelter staff, volunteers, and external agencies.
- Excellent computer proficiency (MS Office – Word, Excel and Google)
Required Qualifications:
Employment Eligibility Verification:

Negative Tuberculosis Test:
- Must provide documentation of a recent negative tuberculosis (TB) test result to ensure a safe and healthy work environment.
Background Screening:
- Successful completion of a comprehensive background screening, which may include criminal record checks, employment history verification, and reference checks.
Pre-Employment Drug Screening:
- Must pass a pre-employment drug screening to ensure a drug-free workplace. A negative drug test result is required.
Driving Skills:
- Must meet the requirements set by our auto insurance carrier.

As a case manager, you will play a pivotal role in the lives of our residents, taking a person-centered approach to ensure their well-being and progress.Your responsibilities will include conducting thorough assessments and developing personalized care plans that encompass various aspects of their lives, such as mental health treatment, legal representation, and housing stability. Meanwhile, the house manager will focus on creating a safe and supportive living space, equipped with crisis intervention skills to handle challenging situations effectively. Collaborating as a team, you and the house assistant will advocate for the residents, connecting them with crucial services like counseling, substance abuse treatment, and job placement assistance. Your expertise in legal regulations will be vital as you navigate the complexities of the criminal justice system, ensuring fair treatment and due process for the residents.
