Full Job Description

Job Title: Care Coordinator

Reporting Supervisor: Site Lead

A key administrative role on the client care team, individuals in the Care Coordinator role work closely with CORE Health and outside system providers and CORE Health Clients to ensure each client is receiving the necessary care, oversee clinician and staff caseloads, and work with clients to identify and mitigate barriers to care. In this role, Care Coordinators support their assigned zone and Zone Leader(s) by actively engaging with clients, scheduling appointments, and coordinating care across systems. Because of the time Care Coordinators are able to dedicate to the data analytics of client care, staff in this role are uniquely equipped to assist clinicians in the oversight of client care.

Typical Work

Actively engages with clients to schedule and reschedule appointments based on client needs;
Manages caseloads within assigned Zone(s) through overseeing client assignment in coordination with AIS, building new clinician and staff caseloads tracking scheduling and client engagement, notifying clinicians of needed discharges for failure to engage, and ensuring that caseload assignments are maintained in the organizations electronic health record system;
Provides weekly client care recaps detailing client care for the week prior to the Administrative Programs Manager, Zone leader and the clinician including but not limited to, client level of care, the number of clients seen, clinician encounter rates, the amount of active clients and missing or expired documents;
Maintains accurate and complete client tracking records for all clinical documents with an expiration date including but not limited to, treatment plans, crisis plans, PHQ-9 Forms, daily living assessments (DLA-20) and release of information (ROIs);
Communicates with each client and applicable schools on staff caseloads when clinicians are out of the office. Assists clients in rescheduling their canceled appointment;
Conducts re-engagement efforts with clients;
Facilitates the beginning steps of the discharge process on behalf of clinicians including mailing out ten day discharge letters;
Provides clerical support by assisting with scanning documents into the organization’s electronic health record (EHR) system;
Assists client care teams in the coordination of care through processes including but not limited to internal referrals, outside provider referrals, the PointClickCare notification process, and ensures relevant information gets passed on to the appropriate staff;
Completes client reassignment processes helping to ensure continuity of care;
Engages with assigned Zone Leader(s) providing recommendations to actively support caseloads, client assignments, scheduling needs, and other administrative Zone oversight;
Provides Care Coordination coverage for other zones as assigned;
Complies with all HIPAA and 42 CFR privacy and security regulations;
Performs other work as required.

Qualifications

Communicate effectively, orally and in writing, in both group and individual settings;
Ability to work in a team based environment;
Knowledgeable in HIPAA and 42 CFR regulations;
Implement quality process initiatives by utilizing tools to ensure customer satisfaction;
Establishes and maintains effective interpersonal relationships with customers, peers, management, partners, community based organizations, private employers, and other state & public agencies;
Performs computer applications & internet operations;
Ability to sit at a computer for prolonged periods of time;
Ability to lift up to 25 pounds at a time;
Works in a fast paced environment demonstrating flexibility and ability to adapt to change; and,

Recruits & engages customers into a system that provides a multitude of services to meet their needs.
