Social Services / Companion Care Coordinator

- Full-Time, Hourly, Non-Exempt, Weekdays and Every Saturday (40 hours)

JOB SUMMARY:

Responsible for assisting the Director of Social Services in identifying, providing/arranging medically related social services and counseling to assist the residents of The Mayflower to attain or maintain their highest practicable physical, mental, and psychosocial well-being during their stay in the nursing home and in planning for discharge.

Essential Duties & Responsibilities for a Social Services / Companion Care Coordinator:

ADMINISTRATIVE FUNCTIONS

a. To maintain a caseload and deliver Social Services within the Health Center in compliance with state and federal regulations.

b. To participate in the QIS survey process in the Health Center.

c. To facilitate care giver and grief support groups as needed.

d. To maintain contact information and current certification status for all private duty CNA’s employed to work in the Health Center.

e. To maintain clothing and special item catalogs for availability of the resident and/or their family for ordering special items for the resident.

f. Performs the duties of the Director of Social Services in her/his absence.

g. Maintains professional status by periodic educational and professional activities as required by law.

h. Performs other related duties as required to fulfill the objectives of the Mission Statement.

COMMITTEE FUNCTIONS

a. Serves on committees as directed by the Director of Social Services.

SOCIAL SERVICE FUNCTIONS: For the assigned caseload:

a. To participate in the MDS assessment process.

b. To identify resident and family concerns relative to discharge planning, adjustment to illness or setting, and actively initiate and participate in Interdisciplinary care conferences

c. To provide counseling to families and residents and emotional support/orientation and/or make appropriate referrals to community-based and/or in-house providers.

d. To participate in the admission and/or preadmission process, as appropriate

e. To document a resident centered comprehensive social assessment, social history, and progress notes for Health Center residents on caseload that identify resident’s support system, strengths and needs, as well as goals and actions to assist resident in achieving highest practicable level

f. To provide end of life support, education, grief/bereavement counseling to residents and families

g. To provide discharge planning and post discharge referrals for Health Center residents

h. To assist the resident or family with the preparation of and/or understanding of Advance Directives.

i. To welcome new residents and act as liaison with interdisciplinary team, including problem solving, complaint resolution and adjustment to setting.

j. Works directly with Health Center Residents, their relatives, family and friends by aiding in their adjustment, handling their complaints, understanding their behavior and meeting their needs.

k. Makes daily rounds through the Health Center, visiting residents and families and making observations regarding resident/family adjustment, and determining discharge needs.

l. Assists Independent and Assisted Living residents with resources and ancillary services.

m. Will observe for changes in psychosocial needs and possible transition to different level of care.

n. Conduct weekly transition meetings

o. Works closely with in-house psych services to arrange for referrals and monthly meetings

p. Assist independent living residents with completing Advance Directives and coordinate the necessary resources to ensure these documents are in place.

q. Provide resources to independent living and assisted living residents and facilitate educational and support groups based on identified resident needs to promote wellbeing, education, socialization, and community engagement.

COMPANION CARE COORDINATION RESPONSIBILITIES

Serve as the companion care coordinator by assisting independent living and assisted living residents and families with identifying companion care needs, coordinating companion care services through our preferred provider, and serving as a liaison between residents, families to support residents in maintaining their highest level of independence and quality of life.

STAFF DEVELOPMENT FUNCTIONS

a. To provide In-service education regularly to small or large groups on requested topics.

b. To pursue education to increase the Social Services / Companion Care Coordinator’s knowledge in the area of Social Services and the nursing home environment.

HEALTH CENTER RESIDENTS’ RIGHTS FUNCTIONS

a. Assists in ensuring that each of the Health Center residents, or the residents’ legally appointed guardian, is furnished with a copy of the Residents’ Bill of Rights.

b. Maintains confidentiality of all resident information.

c. Assures that the residents’ rights to fair and equitable treatment, self- determination, individuality, privacy, property, and civil rights (including the right to register complaints) are well-established and maintained at all times.

OTHER

a. Performs other duties as assigned by the Director of Social Services.
