Full Job Description

JOB DESCRIPTION:

24/7 caregiver team needed for medically fragile male client. Client has kidney, heart, and lung failure, asthma, fall risk, and currently uses a wheelchair.

Family is dissatisfied with current agency due to caregiver turnover, reliability issues, hygiene/cleanliness concerns, missed appointments, and caregivers sleeping during night shifts. This case requires reliable, proactive, clean, detail-oriented caregivers who follow the care plan, read the appointment calendar, document carefully, and anticipate needs because the client does not like to ask for help. Night caregivers must remain awake and alert. Day caregivers must be active with ADLs, meals, appointments, PT exercises, light housekeeping, and outings.

This is not a passive companion case. It requires active care, strong documentation, clean habits, appointment follow-through, and excellent communication with office and family.

Available shifts are: Monday to Sunday (8am-8pm & 8pm-8am)

Compensation: Competitive pay rate with bonus opportunities through a variety of incentive programs.

Primary Responsibilities:

Assist with all activities of daily living (ADLs), including toileting, dressing, bathing, grooming, transfers, and mobility.
Provide safe wheelchair assistance and support with fall prevention. Assist with physical therapy exercises as directed and encourage mobility.
Provide transportation to medical appointments and coordinate with in-home physical therapists as needed.
Monitor and follow the client's appointment calendar to ensure all appointments are attended on time.
Assist with medication reminders and monitor blood pressure as instructed.
Support the client's use of oxygen equipment, nebulizer (client operates independently), and other medical equipment as needed.
Prepare meals according to dietary guidelines, cut food into small pieces, remind the client to chew thoroughly, and sit with him during meals if requested.
Follow strict food safety practices by washing hands before food preparation, labeling prepared foods with dates, discarding leftovers according to family guidelines, and never serving expired food.
Encourage hydration and proactively offer food, water, and assistance without waiting for the client to ask.
Maintain a clean, organized, and sanitary home environment, including regular dusting, vacuuming, mopping hardwood floors, and disinfecting frequently touched surfaces to help minimize asthma triggers.
Remain awake, alert, and attentive during all scheduled shifts, including overnight care.

Requirements:

Minimum of 1 year PAID in-home experience (personal hygiene and transfer skills)
High School Diploma or GED
Ability to pass a comprehensive drug screen
Ability to pass a DOJ/FBI background clearance
Current TB test, CPR Certification
Means of own reliable transportation
Clean DMV driving record
Valid driver’s license and current auto insurance
Adhere to HIPAA and maintain client confidentiality
Proof of eligibility to work legally in the United States and the ability to speak, read and write fluent English
