Full Job Description

Position Summary
The DPCS Designee assists the Director of Patient Care Services (DPCS) with the coordination, supervision, and oversight of hospice clinical services. In the absence of the DPCS, the DPCS Designee assumes designated responsibilities to help ensure continuity of clinical operations and compliance with applicable federal and California hospice requirements.

Essential Duties and Responsibilities

Serve as the designated backup for the DPCS during the DPCS's absence or unavailability.
Assist with oversight and coordination of hospice patient care services.
Ensure patient care is provided according to the Plan of Care and applicable hospice regulations.
Assist with supervision of nursing staff, LVNs, hospice aides, and other clinical personnel as appropriate.
Review clinical documentation for completeness, accuracy, timeliness, and compliance.
Assist with admissions, recertifications, discharges, transfers, and changes in patient condition.
Coordinate with physicians, nurse practitioners, interdisciplinary group (IDG) members, facilities, patients, and families.
Participate in IDG meetings and ensure appropriate follow-up on patient care needs.
Assist with staff scheduling and ensure adequate clinical coverage for hospice patients.
Assist with orientation, education, competency evaluation, and performance monitoring of clinical staff.
Monitor compliance with physician orders and individualized Plans of Care.
Participate in Quality Assessment and Performance Improvement (QAPI) activities.
Assist with infection control, incident reporting, complaints, grievances, and corrective actions involving clinical services.
Maintain patient confidentiality and ensure compliance with HIPAA requirements.
Assist with preparation for CDPH, CMS, and accreditation surveys and audits.
Immediately report significant clinical, compliance, staffing, or patient-safety concerns to the DPCS and Administrator.
Assume designated DPCS responsibilities when the DPCS is unavailable, within the individual's qualifications and scope of practice.
Perform other clinical leadership duties assigned by the DPCS or Administrator.

Qualifications

Current and active California Registered Nurse (RN) license.
Meets applicable California requirements for the position and responsibilities assigned.
Hospice, home health, or related clinical experience preferred.
Knowledge of hospice Medicare Conditions of Participation and California hospice regulations.
Strong clinical judgment, leadership, organizational, and communication skills.
Ability to supervise clinical personnel and coordinate interdisciplinary patient care.
