Full Job Description

Position Summary

We are seeking a dedicated and compassionate Housing Navigator to join our dynamic team. In this vital role, you will serve as a guiding resource for CalAIM Community Supports program by assisting eligible Medi-Cal members experiencing homelessness or severe housing instability in obtaining and maintaining safe, stable, permanent housing. This position delivers intensive, client-centered support including housing assessments, navigation searches, landlord engagement, benefit linkage, and cross-system care coordination

Core Responsibilities

Housing Navigation & Placement: Assist clients in identifying, securing and maintaining permanent housing. Conducting housing assessments and develop individualized, realistic housing stabilization plans. Support clients step-by-step with housing applications, securing identity documentation, and meeting strict eligibility requirements. Match clients to appropriate housing units.
Landlord Engagement & Market Development: Build and maintain posiitive, lasting relationships with regional landlords and property managers. Negotiate complex lease terms and advocate strongly on behalf of program participants. Maintain an organized database of housing resources and available vacancies, conducting community outreach to expand local inventory.
Multidisciplinary Care Coordination: Collaborate directly with Enhanced Care Management (ECM) teams, healthcare providers, and community partners. Active participation in case conferencing and multidisciplinary team meetings. Coordinate key supportive services indlucing mental health, substance use treatment, and medical care.
Client Support & Advocacy: Provide trauma-informed, culturally responsive services to all clients. Advocate for clients to overcome historical housing barriers such as poor credit, past evications, or legal issues. Assit with linking clients to vital social services benefits.
Documentation & Compliance: Maintain accurate, thorough, and timely case documentation inside the EHR system. Ensure complete adherence to Medi-Cal, CalAIM, HIPPAA, and internal compliance benchmarks, tracking and reporting on housing placement and retention metrics.
Crisis Intervention: Respond rapidly to emerging housing crisis, including risk of immediate eviction or a return to active homelessness, and connect clients to local emergency resources when required.

Qualification & Requirements

Education: Bachelor's degree in Social Work, Human Services, Public Health, or a related field required (equivalent direct field experience may be accepted).
Experience: Minimum of 2 years of professional experience working directly with homeless or high-risk, high-acuity populations. Direct experience working within CalAIM frameworks or Medi-Cal programs is highly preferred.
Knowledge & Skills: Solid knowledge of localized housing resources, including HUD, Section 8, and specialized local housing authority programs. Strong understanding of trauma-informed care and harm reduction principles. Excellent communication, negotiation, problem-solving, and time mangement skills. Ability to build swift rapport with diverse populations and operate effectively both independently and within multidisciplinary teams. Bilingual fluency in Spanish is preferred.
Physical Requirements: Ability to travel frequently within the designated regional service area. Hold a CA drivers license and proof of auto insurance. Ability to safely lift and carry up to 20 pounds. (files, outreach material, etc.
