Full Job Description

Housing Stability on-site position ($57,288 -$60,000) equivalent to ( $29.37 to $30.76)

Position Summary:

The Housing Stability Advisor will conduct assessments, determine eligibility, develop individualized case plans, provide landlord/tenant mediation, link families with community-based housing and service resources, and manage financial assistance funds (i.e., rental arrears, utility arrears, security deposit, rental assistance) along with case management services in conjunction with community partners in order to support clients in their financial stability, prevent homelessness. This position will provide intensive case management services, emergency financial assistance and intervention as well as resources and training regarding housing and eviction prevention to households deemed eligible to receive services. Will collaborate with financial coach and support case managers in providing wraparound services to best support each household.

Essential Duties:

Review eligibility intake and comprehensive assessment for each family to determine housing needs, service/support needs.
Develop and implement an individual Customer Service Plan for household in collaboration with clients to address barriers and establish action steps to achieve short and long-term goals.
Will support families in the areas of, but not limited to: housing related services, housing stabilization, community integration, employment linkage, benefits establishment, linkage to community providers for substance abuse, primary and mental health care, and all other services needed to assist clients in reaching their customer service plan goals.
Provide referrals as well as take referrals from legal services with the goal of preventing evictions.
Conduct housing inspections to ensure they meet habitability standards.
Accompany families to housing appointments, assist families in their housing search, housing location services and assist with housing documentation, as needed.
Facilitate access to services required to foster housing readiness (credit repair, legal aid, housekeeping, money management, tenant rights and responsibilities, etc.) in collaboration with financial coach.
Leverage housing location services, to identify housing resources and develop relationships with property managers, property management companies, and landlords.
Provide information and instruction to clients regarding how to complete a housing application, housing search, tenant rights and responsibilities, including: observation of rental agreement rules, and being a good neighbor, especially when needing to relocate from current housing.
Conduct crisis and risk assessments in consultation with supervisor/clinical staff.
Maintain documentation standards as set forth by the program contract and El Nido Family Centers’ policies. Collect all necessary documentation needed to support assistance provided
Find temporary housing opportunities for eligible participants and identify permanent housing options for program participants
Prepare requests for financial assistance and submit all required documentation to supervisor for approval
Manage and track financial assistance funds in collaboration with Supervisor to compare accounting quarterly reports.
Provide appropriate resources, tools, and counseling to assist participants in achieving their case plan goals; making referrals for services and ensuring that the referrals were completed; and tracking and documenting participant progress.
Work closely with other agency programs or departments to maximize participant outcomes, program goals and agency mission; i.e., work collaboratively with case managers to assist participants in accessing housing and related resources
Maintain close collaboration and communication with formal partner agencies for the program (Legal Aid, Work Source Centers, DPSS, family shelters, etc.)

Requirements:

B.A. degree in Social Work or related field.
Minimum of two years’ experience in housing case management, mental health and permanent supportive housing experience required.
Experience providing homeless services, case management, mental health , and permanent supportive housing experience
Flexible work schedule to include some evenings and/or weekends as needed
Possesses a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs
Strong written and verbal communication skills
Highly motivated self- starter with the ability to coordinate multiple projects/ tasks simultaneously in a high-pressure, fast paced environment and work independently
Bilingual (Spanish and English) proficiency required.
Proficient use of computers, Microsoft Office and any other database software(s) used to track service delivery
Knowledge of community resources available in various neighborhoods, particularly those in SPA 6/South Los Angeles.
