Full Job Description

Summary

The Lead Case Manager, reports to the GRYD Coordinator, to conduct outreach and a referral network for project services; plan, develop and implement diversion activities; Youth Development Activities, behavior modification, and client advocacy services to participants including rap groups, individual counseling, family counseling, and probation hearings for project participants; provide support services and charts for project participants. The Lead Case Manager leads a team of 4 to motivate, coach, mentor and provide support to ensure program outcomes and goals. This lead case manager position is able to make independent decisions without close supervision by the Coordinator.

Role and Responsibilities

1. CONDUCT OUTREACH, ADVOCACY AND A REFERRAL NETWORK FOR PROJECT SERVICES (10%):

a. Contact service providers and school personnel in contracted areas and provide information regarding project services.

b. Provide referrals for youth and families to services not encompassed within the project.

c. Communicate with sensitivity to clients from diverse language and cultural backgrounds.

d. Evaluate calls and walk-in's from potential clients and determine appropriateness for GRYD.

e. Prepare all documentation on compliance with Federal, State, City and Agency guidelines.

f. Conduct all screening, intake, contract management and contract eligibility forms for prospective project participants.

g. Create treatment plans/"contracts" for program participants.

h. Provide support and advocacy services for program participants including the attending of probation hearings, school meetings, court and DCFS.

i. Able to work a flexible schedule including weekends and evenings.

j. Conduct other administrative duties as assigned.

2. PLAN. DEVELOP, AND IMPLEMENT YOUTH DEVELOPMENT ACTIVITIES (50%):

a. Contact direct services to youth and their families who are eligible youth between the ages of 10-15 years old by implementing the GRYD Prevention Model of Service throughout the seven phrases.

b. Plan and provide field trips, social and recreational activities, and educational outings for program participants.

c. Develop and maintain strategies for client referrals for the team.

d. Plan community functions i.e. health fairs, resource fairs, community events, etc.

e. Provide transportation on as needed basis

Qualifications and Education Requirements

1. REQUIREMENTS:

Two (2) years of college education related to field or two (2) year- equivalent work experience related to position.
AOD Counselor certification or register with a State approved alcohol or drug counseling certifying organization within thirty days (30) of hire.
Demonstrated knowledge and skill in computer operations.

2. PREFERRED QUALIFICATIONS:

Bilingual or bicultural capabilities with Los Angeles County threshold languages: Arabic, Armenian, Cantonese, Farsi, Urdu, Khmer (Cambodian), Korean, Mandarin, Russian, Spanish, Tagalog, Vietnamese, and other Chinese languages.
Certification in drug and alcohol counseling from state approved organization
Experience in building maintenance
Experience in group facilitation

General Requirements and Knowledge, Skills, and Abilities (KSA's):

Permanent resident or U.S. Citizen; proof of work eligibility upon hiring.
Must have Valid Class C Driver's license, California minimum Automobile Insurance and an appropriate motor vehicle.
Ability to drive 15 passenger vans or obtain a Valid Class C Driver's license with ability to drive 15 passenger vans within 90 days of hire.
Complete physical examination and TB test upon hiring.
Must have three (3) years of recovery or drug free lifestyle as of the date of the application.
Must provide proof of full vaccination and 1st Booster against Covid-19 upon hiring.
Ability to get along well with others, be punctual, pleasant, and tactful with professional appearance and conduct.
Ability to establish and maintain effective working relationships with supervisors, coworkers, clients and their families, community organizations, and the general public.
Ability to be culturally competent of age groups, gender identity, sexual orientation, spiritual beliefs, socioeconomic status, and diverse cultures.

Physical Requirements:

Moderate Duty

This class requires that the incumbent stand or walk most of the time with bending, stooping, squatting, twisting reaching, working or irregular surfaces, occasional lifting of objects over 25 pounds, and frequent lifting of 10-25 pounds. Ascending and descending ladders and staircase will be more frequent. An example is this position frequently will be moving and organizing agency supplies.
