About us

West Valley Adult Day Health Care is a Community Based Adult Services(CBAS) program that looks after the well-being of adults and provides an array of mental and social services, designed to maximize the independence of those who are experiencing significant problems with their health and mental abilities. By enrolling in the program, participants can improve or maintain their physical mobility and function

West Valley Adult Day Health Care Center (ADHC) in Woodland Hills, CA is looking for Masters Level Social Worker) to work on a Full Time basis.

*WORK HOURS: 5 days per week, 5-8 Hours of Work

M-F starting at 8:00 am, Full Time or Part-Time (we are flexible)

Interviewing and screening all applicants/referrals to determine their appropriateness for the full assessment process and participation in the adult day health care program.
Providing referrals for persons not appropriate for adult day health care.
Attending and participating in all the multidisciplinary team (MDT) meetings.
Developing the plan of care in collaboration with the participant’s identified goals and/or concerns.
Completing, at a minimum, quarterly and six-month reassessments which includes evaluating and reviewing the progress made towards the participant’s identified goal(s) collaboratively with participant and updating the plan of care as needed or desired by the participant and the collaborative process.
Conducting an assessment of each participant’s home environment.
Preparing an individual plan of care for each participant, if social services are to be provided.
Providing a treatment program to participants, including counseling and group discussions to address identified problems as specified on the participant’s plan of care.
Leading participants in problem-oriented discussion groups and task-oriented committees.
Providing referrals as needed to available community resources.

Skills

Candidate shall meet one of the following:

1. The person holds a master’s degree in social work from an accredited school of social work;

2. The person holds a master’s degree in psychology, gerontology or counseling from an accredited school and has one year of experience providing social services in one or more of the fields of aging, health, or long-term services;

3. The person is licensed by the California Board of Behavioral Sciences; or

4. The person holds a bachelor’s degree in social work from an accredited school with two years of experience providing social work services in one or more of the fields of aging, health or long term care services.

In addition, the candidate shall:

1. Able to speak, read and write English fluently. Additional Languages a Plus(FARSI)!

2. Physically capable both of performing the required duties and of assisting frail elderly and disabled adults, as necessary.

3. Provide the center with a health examination, signed by a physician, and a TB clearance, which is not more than 12 months prior to employment or within 7 days of employment and shall provide the center a current certificate of CPR and First Aid training.
