Full Job Description

The Child and Family Specialist (CFS) works specifically with the children and family in their home, school environment and respective community. They are responsible for working with adults and children in home and out-of-home placements and their families to provide a variety of services ranging from behavior management, transportation, respite care, school aid, crisis intervention, self-help skill building, vocational training and other services as needed to help them achieve permanency.

ESSENTIAL DUTIES

Carries a caseload and provides case management services to clients and their families including resource linkage and referral and system of care navigation
Work closely with program staff, providers, and community partners to support the family and address their needs towards achieving individual/family goals through weekly & monthly meetings, program events, support groups, etc.
Provides direct services to clients and their families as identified/documented on the client care plan in multiple environments, including but not limited to schools, homes, group homes, and in the community
Provides mental health services, including rehabilitation and collateral and other services such as, behavior management, transportation, respite care, school aid, crisis intervention, safety planning, self-help skill building, vocational training, etc. to clients and their families as deemed necessary and appropriate through the assessment process and in collaboration with the client
Advocate for needs of clients and families to ensure their safety, overall wellbeing, and placement stability
Ability to appropriately document services provided to clients and billing of services per DMH policy
Attend community meetings/conferences and interagency meetings to represent agency/program in a professional manner as assigned by Program Manager
Be available to staff/clients during work and/or after hours for crisis/case consultation (when assigned by Program Manager)
Adheres to HIPAA requirements and regulations for all program participant
Attends all CARF and agency trainings
Adheres to Agency Policies and Procedures

POSITION REQUIREMENTS
To perform effectively in this position, the individual must have:

Bachelor Arts in Human Services, Social Work or Psychology and at least one-year experience working with high risks youth
Knowledge, understanding and skills in working with socially and emotionally disturbed adolescents in a residential or outpatient setting including but not limited to probation youth, mental health and social services programs
Ability to organize, plan and prioritize tasks
Possess good judgement and able to work independently and as a team
Possess good oral and written communications skills to communicate and document work activities
Possess ability to relate to children and caregivers and help support families in working with schools, county agencies and other health care and social service providers
Possess training and adhere to child and adult dependent abuse reporting procedures
Knowledge of community resources, especially related to mental health, education, medical and other social service sectors
Knowledge and ability to provide specific services as required by Contract
Operate a personal computer and routine word processing skills
Complete ongoing agency and DMH trainings as needed
Provide a copy of academic transcripts and degree (s) received
A fingerprint check for criminal history background and must obtain clearance from a law enforcement agency.
Provide copies of valid driver’s license and proof of current auto insurance
Evidence of entitlement to work in the United States in accordance with provisions of the Immigration Reform and Control Act.
Effective oral and written communications skills and ability to read, write, speak, and understand English. Additional language abilities preferred.
Tuberculosis Screening Test one year prior to commencement of employment and as is necessary per Employee Handbook
