Full Job Description

**KIPDA is a part of the Kentucky state health insurance system, the Kentucky Retirement System, and the Kentucky Deferred Compensation Authority. POSTION DESCRIPTION

Class Title: Aging and Disability Resource Center (ADRC) Nutrition Assessor

Division: Social Services/Area Agency on Aging and Independent Living (AAAIL) ADRC

Supervisor: ADRC Coordinator

Supervises: None

Class Characteristics: Under general direction, this position is responsible for telephone and walk-in screening and/or assessments of individuals to determine program eligibility for appropriate long-term care options and nutrition services; provides information and resources.

General Duties and Responsibilities:

Essential:

1. Assist, educate, and support individuals who walk into or call the ADRC attempting to negotiate the aging and disability network or coping with difficult long-term care decisions, about available community resources and services.

2. Complete telephonic, or in-person community-based home visits and computerized screenings for seniors requiring assessments, to determine an individual’s appropriateness for starting or continued services administered within KIPDA region.

3. Refer seniors and all seeking assistance from ADRC through the “No Wrong Door” model to community agencies as appropriate.

4. Assist clients with navigating crisis situations by offering support.

5. Serve as a referral liaison between community members on communicating consumer related issues and determine through conversation with consumers or referral sources available to aging adults, disabled citizens, and their caregivers.

6. Maintain base working knowledge on all KIPDA programs, Medicare, Medicaid, long term care topics and other senior issues to aid seniors, individuals with disabilities and their caregivers.

7. Attend trainings and in-services to obtain and or maintain certification for State Health Insurance Program (SHIP), Alliance on Information and Referral Services (AIRS) or other programs as recommended by the ADRC Coordinator.

8. Fulfill customer service role providing a welcoming atmosphere and ensures customer satisfaction is fulfilled.

9. Ability to work independently to meet established goals.

10. Utilize date entry systems as applicable and assure accurate timely reporting: SAMS, ServTracker, MWMA, UniteUs, to collect client data and process assessments/reassessment and or applications.

11. Maintain knowledge of and practice client confidentiality in accordance with state, federal and KIPDA set HIPPA/HITECH and quality assurance policies and procedures.

12. Collaborate with KIPDA staff and ADRC Coordinator on resource database updates as needed.

13. Work flexible schedule to meet community outreach and networking events as needed.

14. Other duties as assigned.

DESIRABLE QUALIFICATIONS

Training and Experience: Graduation from high school or equivalent is required; bachelor’s degree preferred supplemented by a minimum of one-year of experience in health care, community outreach, community resources, medical social work, geriatrics, or providing individuals with information about long-term care options or related information.

Special Knowledge, Skills, and Abilities:

Knowledge:

1. Knowledge and understanding of issues related to older adults and disabled population.

2. Knowledge of community resources in Louisville and the KIPDA region is preferred.

Skills:

1. Excellent oral and written communication skills. Ability to compose reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

2. Excellent computer skills, including MS Office 365, data entry, and the ability to learn new software applications, as needed.

Abilities:

1. Ability to be interview individuals with tolerance and empathy.

2. Ability to respect cultural, spiritual, racial, and ethnic beliefs and/or lifestyle choices of others; supports and manages diversity inside the KIPDA team.

3. Ability to work as a team; builds cooperation and treats team members equitably.

4. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

ADDITIONAL INFORMATION

Instructions: Very general; uses critical thinking and own judgment.

Processes: Policies and procedures are updated regularly and require an ability to adapt to new, different, or unusual approaches to complete work.

Review of Work: Supervisor regularly reviews work.

Analytical Requirements: Duties are of a complex nature, requiring judgment for which there is no precedent.

Tools, Equipment and Vehicles Used: Normal office equipment (computer, copier, telephone, fax machine, etc.).

Physical Requirements of the Job: Work is typically performed while in various settings such as the community, program participants homes, or sitting at desk or table with intermittent standing, stooping, and walking; lifting light objects (less than 25 pounds) is a job requirement; must operate motor vehicle.

Contacts: Frequent public and private contacts requiring tact and diplomacy.

Confidential Information: Regular use of confidential information.

Mental Effort: Moderate to heavy.

Interruptions: Constant.

Special Licensing Requirements: None.

Availability: N/A

Certification Requirements: AIRS Certification within 2 years of hire date or selection for the ADRC position, Basic SHIP training within 3 months of hire date, ADRC basic training within 3 months of hire date, and maintain all state and KIPDA required continuing education and training
