Full Job Description

Job Overview

We are seeking a dedicated and compassionate Intake Therapist to join New Leaf Clinic. In this vital role, you will serve as the first point of contact for clients seeking mental health services, conducting comprehensive assessments and developing individualized treatment plans. This position serves as the primary point of clinical entry into services by completing initial screenings, biopsychosocial assessments, diagnostic evaluations, and initial prior authorization requests.

The Intake Therapist establishes diagnoses, determines the appropriate level of care, develops initial treatment recommendations, and coordinates admissions and referrals based on each client’s assessed needs. This position collaborates with the interdisciplinary treatment team to support timely access to services and continuity of care.

The ideal candidate demonstrates strong clinical assessment and documentation skills, is knowledgeable in DSM-5 diagnostic criteria, ASAM Criteria, LOCUS, and behavioral health prior authorization requirements, and is committed to delivering high-quality, person-centered care. Candidates with prior authorization or utilization review experience are highly encouraged to apply.

Essential Responsibilities

Conduct initial clinical screenings before comprehensive intake appointments to assess presenting concerns, immediate safety needs, eligibility for services, and potential level-of-care needs.
Identify individuals who may require an urgent assessment, crisis intervention, medical evaluation, detoxification, or referral to a higher level of care.
Conduct comprehensive intake assessments, including biopsychosocial evaluations, mental status examinations, risk assessments, diagnostic evaluations, and substance use screenings.
Establish rapport and develop therapeutic relationships with clients throughout the screening, intake, and assessment process.
Utilize DSM-5 criteria to accurately diagnose mental health disorders and substance use disorders.
Demonstrate competency in applying ASAM Criteria and LOCUS to determine the appropriate level of care and develop clinically appropriate treatment recommendations.
Complete initial prior authorization requests during the intake process, including gathering required clinical information, documenting medical necessity, and submitting accurate and timely authorization materials.
Collaborate with administrative, billing, clinical, and utilization review staff to address missing information or other issues that may delay authorization or admission.
Develop initial treatment plans and coordinate referrals to appropriate internal and external services based on assessed needs.
Complete suicide risk assessments, safety planning, crisis intervention, and de-escalation when clinically indicated.
Collaborate with psychiatrists, therapists, case managers, peer support specialists, medication providers, and other treatment team members to ensure coordinated client care.
Communicate assessment findings, diagnoses, level-of-care recommendations, and initial treatment recommendations to appropriate staff while maintaining client confidentiality.
Maintain accurate, timely, and compliant clinical documentation, including screenings, assessments, diagnoses, treatment plans, prior authorization documentation, crisis documentation, referrals, and admission recommendations.
Maintain complete and current client records, ensuring all required consents, releases of information, and clinical documentation are completed and up to date.
Monitor the intake and admission process and assist with coordinating admissions, referrals, and transitions between levels of care.
Follow up on incomplete assessments, outstanding documentation, authorization requests, and referrals as needed.
Participate in case consultations, multidisciplinary staffing, supervision, and quality improvement initiatives.
Remain current on evidence-based assessment practices, behavioral health regulations, licensing standards, payer requirements, and clinical documentation expectations.
Maintain professional boundaries and adhere to ethical standards established by the applicable licensing board.
Participate in agency training, meetings, and other duties as assigned to support organizational goals.
At least one year of experience conducting clinical behavioral health intakes, biopsychosocial assessments, diagnostic evaluations, or similar clinical assessments.
Demonstrated knowledge of mental health and substance use disorders and DSM-5 diagnostic criteria.
Experience completing risk assessments, safety planning, diagnostic assessments, and level-of-care determinations.
Knowledge of ASAM Criteria and LOCUS.
Strong clinical writing, documentation, communication, time-management, and organizational skills.
Ability to manage multiple scheduled assessments, screenings, documentation requirements, and authorization deadlines.
Ability to work collaboratively within an interdisciplinary behavioral health treatment team.
Current clinical license or credential that meets applicable agency, payer, and state requirements.

Preferred Qualifications

Previous experience completing behavioral health prior authorizations, utilization reviews, or medical-necessity documentation.
Knowledge of Medicaid, commercial insurance, and managed-care authorization requirements.
Experience conducting initial eligibility or pre-intake screenings.
Experience working with clients who have co-occurring mental health and substance use disorders.
Familiarity with behavioral health admission, referral, and level-of-care transition processes.
