StarCare Specialty Health System (StarCare) is seeking a full-time MH Rehab/Supportive Housing & Employment Specialist to join its outstanding Behavioral Health team. The MH Rehab/Supportive Housing, and Employment Specialist plays a critical role in supporting adults with serious mental illness who are experiencing homelessness or are at risk of homelessness by providing community-based services that promote housing stability, recovery, self-sufficiency, and community integration. Working collaboratively with participants and their assigned treatment teams, this position assists individuals in obtaining, maintaining, and transitioning to stable housing while delivering psychosocial rehabilitation, employment support, independent living skills training, and resource coordination. Responsibilities include conducting assessments, developing and implementing individualized service plans, facilitating housing navigation, coordinating community resources, providing skills training, supporting employment goals, and monitoring participant progress. The specialist maintains timely and accurate documentation, fosters effective communication with participants, supervisors, team members, and community partners, and ensures services are delivered in accordance with organizational standards and recovery-oriented practices. Through the provision of high-quality, person-centered care, this position contributes to StarCare’s mission of enhancing quality of life and promoting long-term independence.
Responsibilities:

Provide community-based rehabilitation, housing stabilization, employment support, independent living skills training, psychoeducation, and resource coordination services to adults with serious mental illness.

Conduct participant assessments and reassessments; develop individualized service plans; and complete required documentation to identify strengths, barriers, service needs, and progress toward established goals.

Assist participants in securing, maintaining, and transitioning to safe, stable, and affordable housing through housing navigation, advocacy, and coordination of community resources.

Support the development of skills related to budgeting, financial management, employment readiness, tenant responsibilities, problem-solving, independent living, and successful community integration.

Coordinate and collaborate with behavioral health providers, medical providers, case managers, housing agencies, landlords, employers, benefits programs, legal entities, hospitals, and other community partners to enhance service delivery and participant outcomes.

Maintain timely, accurate, and compliant documentation of services provided, including assessments, service plans, progress notes, discharge summaries, and required electronic health record entries in accordance with organizational policies and regulatory requirements.

Foster effective communication with participants, treatment team members, supervisors, and community stakeholders to ensure coordinated, high-quality care.

Participate in interdisciplinary team meetings, case consultations, quality improvement initiatives, and program development activities.

Deliver person-centered, recovery-oriented services that promote participant choice, dignity, self-determination, independence, and long-term success in the community.

Monitor participant progress, evaluate service effectiveness, and modify interventions as needed to support the achievement of individualized recovery, housing, and employment goals.

Maintain regular and consistent attendance for the assigned work hours.

Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, in addition to StarCare policies and procedures.

Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.

Schedule:
This is a full-time, on-site, position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications:

Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field required.

Experience working with individuals with mental health conditions in supportive housing, employment services, case management, psychosocial rehabilitation, or other community-based behavioral health programs preferred.

Knowledge of recovery-oriented practices, psychosocial rehabilitation, community resources, and supportive services preferred.

Strong interpersonal, communication, organizational, and documentation skills required.

Ability to work independently, collaborate effectively with multidisciplinary teams, and engage individuals with complex behavioral health and social service needs.

Proficiency in electronic health record systems and standard computer applications preferred.

Must have a current Texas driver’s license and be insurable under the Center’s insurance.

Must pass all required pre-employment and annual screenings applicable to the position or services provided, as required by law, regulation, or agency policy, which may include criminal background, nursing registry, MVR, and drug screening based on applicable requirements.
