Overview

We are seeking a detail-oriented and highly skilled Medical Records Facility Certified Coder to join our healthcare team. The ideal candidate will possess extensive knowledge of medical coding standards, billing procedures, and electronic health record management. This role is vital in ensuring accurate coding for inpatient and outpatient services, supporting revenue cycle management, and maintaining compliance with regulatory guidelines.

Essential Functions

Review medical records and assign accurate codes for diagnoses and procedures.
Assign and sequence codes accurately based on medical record documentation.
Assign the appropriate discharge disposition to medical records.
Abstract and enter the coded data for hospital statistical and reporting requirements.
Communicate documentation improvement opportunities and coding issues to appropriate personnel for follow up and resolution.
Maintain 95% coding accuracy rate and 95% accuracy rate for MS-DRG assignment and maintain site designated productivity standards.
Maintain minimum production of 1 chart per hour or site-specific productivity standards.
Demonstrate excellent written and verbal communications skills.
Communicate professionally with co-workers, management, and hospital staff regarding clinical and reimbursement issues.
Adhere to company HIPAA policies and procedures, identifying, maintaining and protecting sensitive HIPAA information (PHI) and following procedures to ensure the security of such information.

Qualifications

Possession of a Certified Coding Associate (CCA) or Certified Coding Specialist designation (CCS) issued by the American Health Information Management Association; or
Two years of experience in medical record coding, or the equivalent combination of experience, education, and training that would provide the required knowledge and abilities.
Minimum of 2-5 years of coding experience in a hospital and/or coding consulting role.
Experience in computerized encoding and abstracting software.

Knowledge, Skills, and Abilities

Knowledge of ICD-10-CM and CPT coding guidelines; medical terminology; anatomy and physiology; state and federal Medicare reimbursement guidelines.
Ability to research and analyze data, draw conclusions, and resolve issues; read, interpret, and apply policies, procedures, laws, and regulations.
Ability to read and interpret medical procedures and terminology.
Ability to develop training materials, make group presentations, and to train staff.
Ability to exercise independent judgement.
Ability to communicate effectively.
Excellent written and verbal communication skills to prepare reports and related documents and to maintain working relationships with physicians and other staff.
Work independently, exercise creativity, be attentive to detail, and maintain a positive attitude.
Manage multiple and simultaneous responsibilities and prioritize scheduling of work.
Maintain confidentiality of all staff, medical, financial, and/or legal related information.
Complete work assignments accurately and in a timely manner.
Handle difficult situations involving patients, physicians, or others in a professional manner.
