Full Job Description

THIS ROLE IS HYBRID. MUST RESIDE IN EITHER WAYNE OR MACOMB COUNTY FOR CONSIDERATION.

Monday through Friday
8:00 AM - 5:00 PM
50-70% Travel (Conducting Member Assessments- Milage Reimbursed)
PAY RATE IS $44.00 PER HOUR

Summary

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.

Requirements

An Associate's degree in Nursing
An active and unrestricted RN license in the state of MI
Case Management Certification CCM preferred
3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care
1 year of experience managing chronic conditions, such as diabetes, hypertension, and asthma
Experience preferred managing the CSHCS population (Children's Special Health Care Services)
Proficiency with computer skills which includes navigating multiple systems and keyboarding
Reliable transportation and valid car insurance
